The pathology can affect various organs, with the liver being one of the most susceptible, resulting in a range of clinical manifestations. Objective: The objective of this study is to describe the pathophysiology involved in liver complications during the critical phase of patients with dengue virus infection. Materials and methods: We conducted a systematic literature search in Medline following the PRISMA guidelines, using health descriptors such as MeSH terms Dengue, Gastrointestinal disease, Dengue hemorrhagic, Liver, Severe dengue, and Hepatic manifestations. Results: We reviewed 20 publications between 2005 and 2020. The most frequent types of publications were subject reviews (50%), followed by case reports (25%), clinical studies (15%), and case-control studies (10%). All publications (100%) describe the pathophysiology involved in liver complications during the critical phase of patients with dengue virus infection. Based on these studies, liver involvement in dengue infection can range from subclinical changes in biochemical markers to severe liver diseases, such as acute liver failure. The liver damage results from either direct viral toxicity or dysregulated immune response to the virus. However, the complete pathophysiology of liver involvement in dengue remains unclear. Conclusion: Hepatic involvement in dengue infection is observed in patients with severe clinical manifestations, attributed to the tropism of different DENV serotypes for hepatocytes and Kupffer cells. Patients with liver involvement exhibit symptoms such as hepatomegaly, pain in the right upper quadrant, jaundice, elevated liver enzymes, nausea, vomiting, and liver failure.

Genera cuadro clínicos asintomáticos o sintomáticos como fiebre, cefalea, dolor retro-ocular, mialgias, entre otros. La patología puede incluir diferentes órganos blancos, entre los cuales el hígado es uno de los más susceptibles, causando afectación y variedad de manifestaciones clínicas. Objetivo: Describir la fisiopatología implicada en las complicaciones hepáticas de la fase crítica en pacientes con infección por el virus del dengue. Materiales y métodos: Se realizó una búsqueda sistemática de la literatura en Medline mediantes las pautas de declaración PRISMA con ayuda de los descriptores en salud tipo MeSH Dengue, Gastrointestinal disease, dengue hemorrhagic, liver, severe dengue y hepatic manifestations. Resultados: Se revisaron 20 publicaciones entre 2005 y 2020. El tipo de publicación más frecuente corresponde a revisiones de tema (50 %), seguido por reportes de caso (25%), estudios clínicos (15%) y casos y controles (10%). Del total de publicaciones, el 100% de las publicaciones describe la fisiopatología implicada en las complicaciones hepáticas de la fase crítica de pacientes con infección por el virus del dengue. Con base a estos estudios, se puede envidenciar que el compromiso hepático en la infección de dengue puede variar desde un compromiso subclínico con cambios bioquímicos hasta enfermedad hepática grave como insuficiencia hepática aguda. Esto se da como resultado de una toxicidad viral directa o una lesión inmunológica desregulada en respuesta al virus, sin embargo, la fisiopatología de la afectación hepática en el dengue no está del todo clara todavía. Conclusión: El compromiso hepático dentro de la infección por dengue se puede presentar en aquellos pacientes que presentan cuadros clínicos graves, debido al tropismo por los hepatocitos y las células de Kupffer que presentan los diferentes serotipos del DENV.
