The case analysis includes a clinical history description, patient images, a surgical technique description, and a literature review on anorectal malformations. In conclusion, anorectal malformation proves to be a common congenital anomaly, with sporadic cases being diagnosed in adolescence. In our developing country, various factors contribute to delayed diagnoses, such as misinformation, illiteracy, and poverty. To mitigate complications, achieve favorable postoperative results, and enhance the quality of life for these patients, early identification of ARM cases is crucial. Therefore, it is imperative to educate medical professionals and raise awareness among family members to encourage early consultations.

El concepto de MAR incluye a la totalidad de anomalías caudales congénitas que acompañan al ano imperforado. La gran mayoría de los casos son detectados en el periodo neonatal o en la infancia temprana, sin embargo, existen pocos casos que son diagnosticados en la adolescencia. Este manuscrito tiene como objetivo presentar el caso de una paciente de 14 años con malformación anorrectal con fístula rectovestibular de diagnóstico tardío a quien se le realizó la corrección de su patología mediante anorrectoplastia sagital posterior con buena evolución postoperatoria. Para el desarrollo del caso se presentó la descripción de la historia clínica, imágenes de la paciente, descripción de la técnica quirúrgica además de revisión de la literatura sobre malformación anorrectal. Finalmente se concluyó que la malformación anorrectal es una anomalía congénita frecuente que se diagnostica a temprana edad, sin embargo, existen pocos casos que son diagnosticados en la adolescencia. En un país en vía de desarrollo como el nuestro hay una combinación de factores que contribuyen al diagnóstico tardío como la desinformación, el analfabetismo y la pobreza.
