The criteria used for the selection of articles were studies conducted in humans, in English or Spanish and published between 1995 and 2020. Results: Twenty-two articles published between 1995 to 2020 were included. The majority of studies used some cytoreductive conditioning regimen prior to combined kidney and hematopoietic stem cell transplantation. For chimerism, the majority achieved a mixed or complete chimeric state. The average of immunosuppressive drug withdrawal after renal and hematopoietic stem cell transplantation was 2.4 months, and was maintained for 4 to 55 months, with satisfactory recovery of renal function and no evidence of rejection episodes. Finally, 22 articles included in this review concluded that chimerism has a role in inducing immune tolerance that improves kidney survival in patients undergoing kidney transplantation. Conclusions: Mixed chimerism induces immunological tolerance that translates into stable kidney function, lower rejection rates, and longer kidney graft survival, without the need for maintenance immunosuppressive therapy.

Materiales y métodos: Se realizó una búsqueda sistemática de la literatura en Medline, mediante las pautas de la declaración PRISMA y los descriptores en salud tipo MeSH. Los criterios utilizados para la selección de artículos fueron, estudios realizados en humanos, en idioma inglés o español y publicados entre 1995 a 2020. Resultados: Se incluyeron 22 artículos publicados entre los años 1995 a 2020. La mayoría de estudios utilizaron algún régimen de condicionamiento citorreductivo previo al trasplante combinado de riñón y células madre hematopoyéticas. En cuanto al quimerismo, la mayoría logró alcanzar un estado quimérico mixto o completo. El promedio de retirada de la terapia inmunosupresora después del trasplante renal y de células madre hematopoyéticas fue 2,4 meses y se mantuvo durante 4 a 55 meses, con recuperación satisfactoria de la función renal y sin evidencia de episodios de rechazo. Finalmente, los 22 artículos incluidos en esta revisión, concluyeron que el quimerismo tiene un papel en la inducción de tolerancia inmunológica que mejora la supervivencia renal en pacientes sometidos a trasplante renal.
