description Introduction: The ulnar nerve injury is the second most common neuropathy of the upper limb. It generates sequelae such as palmar atrophy, finger deformity and characteristic clinical signs like clawed hands. However, the motor deficits do not always correspond to the patient’s clinical condition and presentations may vary, like the “ulnar nerve paradox”, which hinders understanding and identifying the injury in clinical practice by modifying the common clinical knowledge of nerve injuries. Objective: To understand the anatomical relations that could be associated with the presence of the “ulnar paradox” in ulnar nerve injuries. Materials and methods: A literature review was conducted in Scopus and Medline using controlled terminology and advanced search equations, managing to retrieve 24 articles about the ulnar nerve anatomy, anatomic variations and motor behavior of the injured hand. Results: The anatomical path of the ulnar nerve is intercepted by multiple structures that can alter its integrity and favor inflammatory states, in addition to being exposed to muscular and nervous anatomical variations such as the “Martin-Gruber anastomosis”; all these are factors that could explain the variety of presentations in hand deformities after injury of the ulnar nerve. Conclusion: The ulnar nerve paradox is a variable, multifactorial and poorly known clinical entity that requires detailed knowledge of the ulnar nerve and the predisposing anatomical factors for its identification and understanding.

Genera secuelas como la atrofia palmar, deformidades en los dedos y signos clínicos característicos como la mano en garra. Sin embargo, sus compromisos motores no siempre corresponden a la clínica del paciente, dando lugar a presentaciones clínicas variables como la llamada “paradoja del nervio ulnar”, las cuales afectan la comprensión e identificación de la lesión en la práctica médica dificultando el diagnóstico médico. Objetivo: Comprender las relaciones anatómicas que puedan estar asociadas a la presencia de la paradoja del nervio ulnar ante su lesión nerviosa.  Materiales y métodos: Se realizó una revisión de la literatura en Scopus y Medline, utilizando terminología controlada y ecuaciones de búsqueda avanzada, recopilando 24 artículos sobre la anatomía del nervio ulnar, variaciones anatómicas y reportes del comportamiento motor de la mano en su lesión.  Resultados: El  recorrido anatómico del nervio ulnar se ve interceptado por múltiples estructuras que pueden alterar su integridad y favorecen estados inflamatorios, además de estar expuesto a variaciones anatómicas musculares y nerviosas como la “Anastomosis de Martin-Gruber”; factores que podrían explicar las presentaciones variadas en las deformidades de la mano posterior a la lesión del nervio ulnar.
