La ferritina y el dímero D, biomarcadores inflamatorios, se relacionaron con la gravedad de la enfermedad y variaron significativamente en pacientes hipertensos con COVID-19 severo. El objetivo de este estudio fue evaluar la ferritina como predictor de morbimortalidad en pacientes hipertensos con COVID-19 severo, comparándola con el dímero D. Método: Se realizó una revisión sistemática siguiendo los lineamientos de la estrategia PRISMA (2020). La búsqueda de literatura se llevó a cabo en las bases de datos PubMed (MedLine), Scopus, Science Direct y Web of Science, incluyendo estudios observacionales de población adulta hipertensa hospitalizada por COVID-19 severo que evaluaron la ferritina y el dímero D como predictores de morbimortalidad entre 2019 y 2023. Los hallazgos se describieron cualitativamente, utilizando la lista de verificación PRISMA (2020) y apoyándose en la plataforma Rayyan AI para la selección de estudios. Resultados: Se obtuvo información detallada sobre la utilidad de la ferritina como predictor de morbimortalidad en comparación con el dímero D. Los datos de los estudios seleccionados incluyeron edad, niveles de ferritina y dímero D. La búsqueda inicial arrojó 4586 resultados, de los cuales se seleccionaron 27 estudios para análisis cuantitativo. El análisis reveló que los pacientes adultos hipertensos hospitalizados por COVID-19 severo presentaron incrementos significativos en los niveles de ferritina y dímero D, indicando su relevancia como biomarcadores. Conclusiones: Esta revisión sistemática proporcionó información crucial sobre el valor pronóstico de la ferritina en pacientes hipertensos con COVID-19 severo. Los resultados sugieren que la ferritina podría ser un biomarcador útil para mejorar el manejo clínico de estos pacientes, complementando la información proporcionada por el dímero D. This study aimed to evaluate ferritin as a predictor of morbidity and mortality in hypertensive patients with severe COVID-19, comparing it with D-dimer. Method: A systematic review was conducted following the PRISMA (2020) guidelines. The literature search was performed in the databases PubMed (MedLine), Scopus, Science Direct, and Web of Science, including observational studies of adult hypertensive populations hospitalized with severe COVID-19 that evaluated ferritin and D-dimer as predictors of morbidity and mortality between 2019 and 2023. The findings were described qualitatively, using the PRISMA (2020) checklist and supported by the Rayyan AI platform for study selection. Results: Detailed information was obtained on the utility of ferritin as a predictor of morbidity and mortality compared to D-dimer. Data from the selected studies included age, ferritin levels, and D-dimer levels. The initial search yielded 4586 results, from which 27 studies were selected for quantitative analysis. The analysis revealed that adult hypertensive patients hospitalized with severe COVID-19 showed significant increases in ferritin and D- dimer levels, indicating their relevance as biomarkers. Conclusions: This systematic review provided crucial information on the prognostic value of ferritin in hypertensive patients with severe COVID-19. The results suggest that ferritin could be a useful biomarker for improving the clinical management of these patients, complementing the information provided by D-dimer.
