El síndrome coronario agudo es la principal causa de morbimortalidad mundial, el 10% de los casos se presenta en ausencia de enfermedad coronaria aterosclerótica obstructiva. El infarto miocárdico con enfermedad coronaria no obstructiva (MINOCA) representa un grupo de condiciones con hallazgos comunes en la arteriografía coronaria y la ausencia de estenosis mayor al 50% . Se describe con mayor frecuencia en mujeres jóvenes, con menos factores de riesgo cardiovasculares tradicionales. Se desconocen las características de la población afectada con MINOCA a nivel regional. Se propone caracterizar la población con MINOCA e identificar factores asociados a desenlaces adversos. Material y métodos: Estudio analítico tipo cohortes que indentificó las diferentes características en pacientes con MINOCA en un centro de tercer nivel de Pereira. Entre enero 1 de 2019 y Diciembre 31 de 2020 se revisaron 1.500 arteriografías coronarias de las cuales 292 cumplieron criterios angiográficos de MINOCA. De éstos, 163 pacientes cumplieron los criterios clínicos y de inclusión. El desenlace primario fue un compuesto de hospitalización por angina o insuficiencia cardiaca, terapia de reperfusión, muerte por causa cardiovascular y muerte por cualquier causa a 6 meses y a 1 año. |

El infarto miocárdico con enfermedad coronaria no obstructiva (MINOCA) representa un grupo de condiciones con hallazgos comunes en la arteriografía coronaria y la ausencia de estenosis mayor al 50% . Se describe con mayor frecuencia en mujeres jóvenes, con menos factores de riesgo cardiovasculares tradicionales. Se desconocen las características de la población afectada con MINOCA a nivel regional. Se propone caracterizar la población con MINOCA e identificar factores asociados a desenlaces adversos. Material y métodos: Estudio analítico tipo cohortes que indentificó las diferentes características en pacientes con MINOCA en un centro de tercer nivel de Pereira. Entre enero 1 de 2019 y Diciembre 31 de 2020 se revisaron 1.500 arteriografías coronarias de las cuales 292 cumplieron criterios angiográficos de MINOCA. De éstos, 163 pacientes cumplieron los criterios clínicos y de inclusión. El desenlace primario fue un compuesto de hospitalización por angina o insuficiencia cardiaca, terapia de reperfusión, muerte por causa cardiovascular y muerte por cualquier causa a 6 meses y a 1 año.Acute coronary syndrome is the main cause of global morbidity and mortality, 10% of cases occur in the absence of obstructive atherosclerotic coronary disease. Myocardial infarction with non-obstructive coronary artery disease (MINOCA) represents a group of conditions with common findings in coronary arteriography and the absence of stenosis greater than 50%. It is more frequently described in young women, with fewer traditional cardiovascular risk factors. The characteristics of the population affected with MINOCA at the regional level are unknown. It is proposed to characterize the population with MINOCA and identify factors associated with adverse outcomes. Material and methods: Cohort-type analytical study that identified the different characteristics in patients with MINOCA in a tertiary care center in Pereira. Between January 1, 2019 and December 31, 2020, 1,500 coronary arteriograms were reviewed, of which 292 met the MINOCA angiographic criteria. Of these, 163 patients met the clinical and inclusion criteria. The primary outcome was a composite of hospitalization for angina or heart failure, reperfusion therapy, cardiovascular death, and death from any cause at 6 months and 1 year. Results: The median age was 64 years and 54% (n=88) were men. Arterial hypertension was the most prevalent comorbidity (n=100; 61.3%), the most frequent electrocardiographic presentation was T-wave inversion (29.7%; n=47) and the majority of cases (n=133 ; 81.6%) had preserved LVEF. 66.3% (n=108) had healthy coronary arteries, 31.9% (n=52) 30-50% stenosis, and 1.8% (n=3) less than 30% stenosis. 19.3% (n=28) and 25.5% (n=37) presented some outcome at 6 months and 1 year, hospitalization for angina being the most frequent. Mortality at one year was predominantly cardiovascular and occurred in 5.5%. In the bivariate analysis, risk factors related to outcomes were identified: male gender, alcohol consumption, diastolic blood pressure, score on scales: GRACE, TIMI, CRUSADE and Killip-Kimball, significant Q wave, moderate to severe aortic or mitral regurgitation, length of hospitalization and initial troponin.
