El trauma cráneo encefálico es una de las principales causas de morbilidad y mortalidad en los países desarrollados y está llegando al punto de ser una de las principales causas en los países en desarrollo, aproximadamente alrededor de 500.000 personas sufren de discapacidad neurológica permanente debido al trauma cráneo encefálico (TCE) y mueren más de 50.000 personas en los Estados Unidos cada año (Mangat, y otros, 2019); debido a la alta incidencia del TCE radica la importancia en la identificación de las lesiones cerebrales secundarias al trauma y el tratamiento oportuno de estás, siendo los incrementos de la presión intracraneal (PIC) una de las principales consecuencias del trauma y debido a esto; el control en la elevación de la PIC y tratar la disminución de la presión de perfusión cerebral son los principios claves en el manejo inicial del paciente neurocrítico. En la actualidad se emplean varias terapias para lograr esto, sin embargo, el uso de la terapia hiperosmótica con solución salina hipertónica a demostrado ser útil como manejo de primera línea en el tratamiento de la hipertensión intracraneal (HIC); sin embargo, la identificación clínica de los cambios en la respuesta pupilar sugestivos de HIC es de limitada interpretación, muy subjetiva y cualitativa visual y más aún representan un problema en la atención pre hospitalaria debido a la situación y el medio en que se encuentra, para esto la implementación de instrumentos automatizados y de fácil manejo como el pupilometro debería ser importante su uso para el diagnóstico objetivo y temprano de los incrementos de la PIC en pacientes que presentan trauma cráneo encefálico (Mangat, y otros, 2019). |
