En nuestro trabajo pretendemos determinar si el tiempo al inicio de la terapia con los antibióticos modifica los desenlaces (choque séptico, necesidad de ingreso a UCI o muerte) de los pacientes con diagnóstico de cáncer y neutropenia febril tratados en la clínica de oncología pediátrica del INC entre 09 de octubre del 2011 al 5 de mayo del 2013; y 01 de octubre del 2014 al 30 de marzo de 2015. Nuestros datos en conclusión no logran demostrar que el inicio temprano del tratamiento antibiótico disminuye la frecuencia de desenlaces adversos, lo que obliga en un futuro a aumentar el tamaño de la muestra, para rechazar o aceptar esta premisa. Es necesario incrementar la búsqueda de los agentes infecciosos mediante la toma de pruebas para diagnosticar virus y mejorar la toma de cultivos en orina y en los sitios de infecciones fuera del torrente sanguíneo. Será necesario en el futuro no solo establecer la asociación entre la demora en el inicio del tratamiento sino también explorar los determinantes de tales demoras para intervenir oportunamente en los servicios de urgencias con miras a mejorar la calidad de atención de estos pacientes. Finalmente debemos declarar que el estudio es un estudio en curso que aún no se ha cerrado

Given the rapid progression of infections in this group of patients, empirical parenteral administration of broad-spectrum antibiotics remains the standard management in the initial treatment of febrile neutropenia in cancer patients. In our work we try to determine whether the time to initiation of therapy with antibiotics changes outcomes (Septic shock, need for ICU admission or death) of patients diagnosed with cancer treated and febrile neutropenia in pediatric oncology clinic INC between October 9 of 2011 to May 5, 2013; and October 1, 2014 to March 30, 2015. Our conclusion data fail to demonstrate that early treatment antibiotics decreases the frequency of adverse outcomes, forcing in a future increase the sample size, to reject or accept this premise. It is necessary to increase the search for infectious agents by taking evidence to diagnose virus and improve decision-crops and urine infections sites out of the bloodstream. It will be necessary in the future not only establish the association between the delay in the start of treatment but also explore the determinants of such delays to intervene timely emergency services in order to improve the quality of care these patients.
