description Este estudio ha pretendido determinar si se presentan cambios en la percepción del volumen del habla y en la conectividad funcional en las redes neuronales (el circuito motor de los ganglios basales y la red de control motor del habla por retroalimentación) asociados a la hipofonía de pacientes con Enfermedad de Parkinson. Para tal fin, se ha realizado un análisis comparativo en las variables de intensidad de la voz, percepción del volumen del habla y conectividad funcional, en las redes neurales mencionadas, entre 13 pacientes con enfermedad de Parkinson y 15 controles sanos. De manera complementaria, se ha analizado una posible relación de la intensidad de la voz con las variables clínicas de nivel de severidad, tiempo de evolución, medicación y lado afectado en los pacientes evaluados. La intensidad de la voz ha sido valorada a través de la Consensus Auditory-Perceptual Evaluation of Voice; el nivel de percepción del volumen del habla se ha obtenido mediante un protocolo consistente en tres tareas: magnitud de la estimación, imitación y magnitud de la producción; la magnitud de conectividad funcional ha sido calculada a partir de un registro de resonancia magnética funcional en estado de reposo. Los resultados han permitido entender la hipofonía como un desorden de integración sensoriomotora. De manera complementaria, se han observado alteraciones adicionales de la voz relacionadas con la aspereza, el escape y la tensión. Con base en tales hallazgos se propone incluir elementos sensoriales auditivos y propioceptivos como parte de la evaluación y el tratamiento de las alteraciones de la voz y el habla secundarias a la enfermedad de Parkinson.

Para tal fin, se ha realizado un análisis comparativo en las variables de intensidad de la voz, percepción del volumen del habla y conectividad funcional, en las redes neurales mencionadas, entre 13 pacientes con enfermedad de Parkinson y 15 controles sanos. De manera complementaria, se ha analizado una posible relación de la intensidad de la voz con las variables clínicas de nivel de severidad, tiempo de evolución, medicación y lado afectado en los pacientes evaluados. La intensidad de la voz ha sido valorada a través de la Consensus Auditory-Perceptual Evaluation of Voice; el nivel de percepción del volumen del habla se ha obtenido mediante un protocolo consistente en tres tareas: magnitud de la estimación, imitación y magnitud de la producción; la magnitud de conectividad funcional ha sido calculada a partir de un registro de resonancia magnética funcional en estado de reposo. Los resultados han permitido entender la hipofonía como un desorden de integración sensoriomotora. De manera complementaria, se han observado alteraciones adicionales de la voz relacionadas con la aspereza, el escape y la tensión. Con base en tales hallazgos se propone incluir elementos sensoriales auditivos y propioceptivos como parte de la evaluación y el tratamiento de las alteraciones de la voz y el habla secundarias a la enfermedad de Parkinson.This study aimed to determine whether there are changes in the perception of speech volume and in functional connectivity in the neural networks (the motor circuit of the basal ganglia and the speech motor control network through feedback) associated with hypophonia in patients with Parkinson’s Disease. For this purpose, a comparative analysis has been carried out on the variables of voice intensity, perception of speech volume and functional connectivity between 13 patients and 15 healthy controls. In a complementary manner, a possible relationship between voice intensity and the clinical variables of level of severity, time of evolution, medication and affected side in the patients evaluated has been analyzed. The intensity of the voice has been assessed through the Consensus Auditory-Perceptual Evaluation of Voice; the level of perception of speech volume has been obtained through a protocol consisting of three tasks: magnitude of estimation, imitation and magnitude of production; the magnitude of functional connectivity has been calculated from a resting state fMRI recording. The results have allowed us to understand hypophonia as a sensorimotor integration disorder. Additional voice disturbances related to harshness, leakiness, and tension have been observed.
