description Introducción: En la UCI existen medidas para prevenir el síndrome postcuidado intensivo (SPCI), un conjunto de secuelas que afectan la salud y la calidad de vida de sobrevivientes y sus familias, dichas estrategias de prevención no siempre son realizadas por el personal de salud que trabaja en cuidado crítico y las razones han sido poco exploradas, así como los conflictos que se presentan en el trabajador por no realizar dichas acciones en beneficio del paciente, como el impacto de la institución hospitalaria frente a la agencia del personal. Objetivo: Comprender la emergencia de los conflictos deontoaxiológicos en un grupo de profesionales de la salud, generados por la tensión agencia y estructura durante las prácticas del cuidado, en el contexto de la prevención del síndrome post-cuidado intensivo. Metodología: A través de la autoetnografía y de veintiún entrevistas semiestructuradas se realizó el análisis cualitativo a través de la fenomenología hermenéutica con el enfoque de seis pasos de Van Manen, siendo la unidad de análisis: los conflictos deontoaxiológicos, que emergen entre la tensión agencia y estructura y la unidad de observación las prácticas del cuidado que realiza el personal de salud en las unidades de cuidados intensivos como estrategias de prevención del SPCI, para responder la pregunta de investigación: ¿Como son los tipos de conflictos deontoaxiológicos que se presentan en un grupo de profesionales de la salud, como producto entre la tensión agencia y estructura durante las prácticas del cuidado, en el contexto de la prevención del síndrome post-cuidado intensivo? Resultados se encontraron seis categorías emergentes: 1. Barreras frente al cuidado en prevención de SPCI compuesta por conocimiento incompleto de los paquetes ABCDEF, educación concentrada otros temas priorizados, fragmentación de los cuidados y relaciones interdisciplinarias escasas. 2. Familia que participa en el cuidado donde se evidenció la presencia y participación de la Familia como factor protector de SPCI, perspectivas diferentes frente a tipos de visitas y heterogeneidad en las Políticas institucionales de humanización. 3. Conflictos con la estructura que se impone sobre el agente, categoría constituída por la sobrecarga laboral, la cantidad de pacientes a atender por profesional, el tiempo y calidad dedicado a actividades con cada paciente, la precarización laboral, tácticas, consecuencias y conflictos en el grupo de trabajo y con los directivos. 4. Categoría Limitación de la agencia: pérdida de derecho a la expresión, sobrecarga de trabajo limita la capacidad frente al cuidado del paciente, condición de cabeza de hogar y género, limita la capacidad de agencia y el desgaste sobre la agencia y pérdida de autoeficacia. 5. "Culpa" como expresión de angustia moral, resaltando en esta categoría el impacto de las emociones en el trabajo en salud, el duelo por el sobreviviente, la saturación responsabilidades, el síndrome de desgaste profesional y el riesgo de fatiga de la compasión-cosificación del paciente. 6. Categoría Agentes Corporados, en tanto agentes unidos que transforman a la estructura, equipos multidisciplinarios, oportunidades con las familias y Educación y Comunicación. Conclusión: Este estudio propone a la UCI como microsociedad y el SPCI como patología social, se reflexiona sobre el hallazgo de la culpa como expresión de conflictividad bioética y angustia moral, así como la importancia de las emociones en el trabajador de salud y su impacto en la agencia frente a las tensiones con la estructura. Cerrando con una propuesta cambio, una utópica propuesta bioética de resolución de este conflicto agencia y estructura con soluciones dialógicas.

Objetivo: Comprender la emergencia de los conflictos deontoaxiológicos en un grupo de profesionales de la salud, generados por la tensión agencia y estructura durante las prácticas del cuidado, en el contexto de la prevención del síndrome post-cuidado intensivo. Metodología: A través de la autoetnografía y de veintiún entrevistas semiestructuradas se realizó el análisis cualitativo a través de la fenomenología hermenéutica con el enfoque de seis pasos de Van Manen, siendo la unidad de análisis: los conflictos deontoaxiológicos, que emergen entre la tensión agencia y estructura y la unidad de observación las prácticas del cuidado que realiza el personal de salud en las unidades de cuidados intensivos como estrategias de prevención del SPCI, para responder la pregunta de investigación: ¿Como son los tipos de conflictos deontoaxiológicos que se presentan en un grupo de profesionales de la salud, como producto entre la tensión agencia y estructura durante las prácticas del cuidado, en el contexto de la prevención del síndrome post-cuidado intensivo? Resultados se encontraron seis categorías emergentes: 1. Barreras frente al cuidado en prevención de SPCI compuesta por conocimiento incompleto de los paquetes ABCDEF, educación concentrada otros temas priorizados, fragmentación de los cuidados y relaciones interdisciplinarias escasas. 2. Familia que participa en el cuidado donde se evidenció la presencia y participación de la Familia como factor protector de SPCI, perspectivas diferentes frente a tipos de visitas y heterogeneidad en las Políticas institucionales de humanización. 3. Conflictos con la estructura que se impone sobre el agente, categoría constituída por la sobrecarga laboral, la cantidad de pacientes a atender por profesional, el tiempo y calidad dedicado a actividades con cada paciente, la precarización laboral, tácticas, consecuencias y conflictos en el grupo de trabajo y con los directivos. 4. Categoría Limitación de la agencia: pérdida de derecho a la expresión, sobrecarga de trabajo limita la capacidad frente al cuidado del paciente, condición de cabeza de hogar y género, limita la capacidad de agencia y el desgaste sobre la agencia y pérdida de autoeficacia. 5. "Culpa" como expresión de angustia moral, resaltando en esta categoría el impacto de las emociones en el trabajo en salud, el duelo por el sobreviviente, la saturación responsabilidades, el síndrome de desgaste profesional y el riesgo de fatiga de la compasión-cosificación del paciente. 6. Categoría Agentes Corporados, en tanto agentes unidos que transforman a la estructura, equipos multidisciplinarios, oportunidades con las familias y Educación y Comunicación. Conclusión: Este estudio propone a la UCI como microsociedad y el SPCI como patología social, se reflexiona sobre el hallazgo de la culpa como expresión de conflictividad bioética y angustia moral, así como la importancia de las emociones en el trabajador de salud y su impacto en la agencia frente a las tensiones con la estructura. Cerrando con una propuesta cambio, una utópica propuesta bioética de resolución de este conflicto agencia y estructura con soluciones dialógicas.Doctor en BioéticaDoctoradoBackground: In the ICU there are measures to prevent post intensive care syndrome (SPCI), defined as a set of sequelae affecting the health and quality of life of survivors and their families, such prevention strategies are not always carried out by healthcare workers in the critical care unit for reasons that have been poorly explored, as well as the conflicts that arise in the worker for not performing such actions for the benefit of the patient, as well as the impact of the hospital in the ministration of staff. Objective: To understand the emergence of deontoaxiological conflicts in a group of healthcare providers, generated by agency and structure tension during care practices, in the context of prevention of post-intensive care syndrome. Methods: Through self-ethnography and twenty-one semi-structured interviews, a qualitative analysis was carried out through hermeneutic phenomenology with Van Manen's six-step approach, the unit of analysis being: deontoaxiological conflicts, which emerge between the tension agency and structure and the observation unit the care practices performed by health personnel in intensive care units and preventive strategies of SPCI, to answer the research question: “What are the types of deontoaxiological conflicts presented in a group of healthcare workers as a result of tension agency and structure during care practices in the context of SPCI prevention? Results: Six emergent categories were found: 1) Barriers in terms of care in SPCI prevention, composed by insufficient knowledge about ABCDEF packages, skills being focused on other priorities, fragmentation of care and scarce interdisciplinary relationships. 2) Family that participates in care, where their presence and participation qualifies as a protective factor of SPCI, different perspectives regarding types of family visits and heterogeneity in institutional policies regarding humanization. 3) Conflicts with the structured imposed on the agent, a category constituted by work overload, number of patients treated per professional, quality time dedicated to each patient’s activities, job precariousness, tactics, consequences and conflicts within the work group and higher ups. 4) Limitation of agency category: loss of the right to expression, work overload limiting the capacity to care for the patient, gender and head of family status, limiting the agency capacity and weathering of the agency and loss of self-efficacy. 5) Guilt, as the expression of moral anguish, highlighting in this category the impact of emotions in healthcare, survivor’s grief, saturation of responsibilities, burnout syndrome and the risk of fatigue of the patient’s compassion-objectification. 6) Corporate agents category, where united agents transform the structure, multidisciplinary teams, opportunities with families, education and communication. Conclusions: This study proposes the ICU as a microsociety and SPCI as a social pathology, reflecting on the finding of guilt as the expresión of bioethical conflict and moral anguish, as well as the importance of emotions in the health worker and their impact on agency in the face of tensions with the structure.
