La Clorhexidina (CHX) es el agente antiplaca más usado para desinfección completa de la boca y en el control de la biopelícula dental en el periodo post operatorio, Sin embargo, algunos efectos colaterales han limitado su uso clínico. El ácido hipocloroso (HOCl) es una nueva sustancia antimicrobiana no antibiótica que ha mostrado alta susceptibilidad sobre bacterias asociada al biofilm dental. Comparar la efectividad clínica y la recolonización bacteriana de la placa subgingival a los 7, 21 y 90 días del HOCl 0.05%, 0,025% y CHX 0.2% y 0,12% durante el periodo post quirúrgico en pacientes con periodontitis crónica. Se realizó un estudio piloto, aleatorizado doble ciego donde se evaluaron 32 pacientes divididos aleatoriamente en dos grupos de 16 cada uno para recibir HOCl al 0,05% durante 7 días, seguido de HOCl al 0,025% o CHX 0.2% seguido de CHX al 0,12% hasta el día 21 después de cirugía periodontal con colgajo de Widman modificado en ausencia de higiene oral. Los pacientes fueron evaluados por índice de placa, índice gingival, profundidad de la bolsa y nivel de inserción y para recolonización bacteriana en placa subgingival por un período de 3 meses. Los datos fueron analizados por ANOVA de medidas repetidas y por modelos de efectos mixtos ajustados a tratamiento, tiempo e interacción tiempo-tratamiento. CHX mostró superioridad para la reducción de placa dental sobre HOCl (p<0.05). Para la reducción de la inflamación gingival, hemorragia al sondaje, reducción de las bolsas y la ganancia del nivel de inserción las diferencias no fueron significativas entre los grupos (p>0.05). En cuanto a la recolonización bacteriana ambos grupos se comportaron de manera similar para P. gingivalis, T. forsithya y T. denticola. A. antinomicentemcomitas y E. nodatum mostraron una rápida recolonización en la placa subgingival y los protocolos antiplaca no parecieron tener un efecto. HOCl muestra efectos similares a CHX en los índices clínicos y en la recolonización bacteriana sin embargo se observaron menos eventos adversos en el grupo del HOCl. El HOCl podría ser equivalente a la CHX como agente antiplaca en periodos post quirúrgicos. |

El ácido hipocloroso (HOCl) es una nueva sustancia antimicrobiana no antibiótica que ha mostrado alta susceptibilidad sobre bacterias asociada al biofilm dental. Comparar la efectividad clínica y la recolonización bacteriana de la placa subgingival a los 7, 21 y 90 días del HOCl 0.05%, 0,025% y CHX 0.2% y 0,12% durante el periodo post quirúrgico en pacientes con periodontitis crónica. Se realizó un estudio piloto, aleatorizado doble ciego donde se evaluaron 32 pacientes divididos aleatoriamente en dos grupos de 16 cada uno para recibir HOCl al 0,05% durante 7 días, seguido de HOCl al 0,025% o CHX 0.2% seguido de CHX al 0,12% hasta el día 21 después de cirugía periodontal con colgajo de Widman modificado en ausencia de higiene oral. Los pacientes fueron evaluados por índice de placa, índice gingival, profundidad de la bolsa y nivel de inserción y para recolonización bacteriana en placa subgingival por un período de 3 meses. Los datos fueron analizados por ANOVA de medidas repetidas y por modelos de efectos mixtos ajustados a tratamiento, tiempo e interacción tiempo-tratamiento. CHX mostró superioridad para la reducción de placa dental sobre HOCl (p<0.05). Para la reducción de la inflamación gingival, hemorragia al sondaje, reducción de las bolsas y la ganancia del nivel de inserción las diferencias no fueron significativas entre los grupos (p>0.05). En cuanto a la recolonización bacteriana ambos grupos se comportaron de manera similar para P. gingivalis, T. forsithya y T. denticola. A. antinomicentemcomitas y E. nodatum mostraron una rápida recolonización en la placa subgingival y los protocolos antiplaca no parecieron tener un efecto. HOCl muestra efectos similares a CHX en los índices clínicos y en la recolonización bacteriana sin embargo se observaron menos eventos adversos en el grupo del HOCl. Hypochlorous acid (HOCl) is a novel non-antibiotic antimicrobial substance that has shown high susceptibility to bacteria associated with dental biofilm. To compare the clinical effectiveness and bacterial recolonization of subgingival plaques at 7, 21 and 90 days of HOCl 0.05%, 0.025% and CHX 0.2% and 0.12% during the post-surgical period in patients with chronic periodontitis. A randomized, double-blind pilot study was conducted in which 32 patients were randomly divided into two groups of 16 each to receive 0.05% HOCl for 7 days, followed by 0.025% HOCl or CHX 0.2% followed of CHX to 0.12% until day 21 after periodontal surgery with modified Widman flap in the absence of oral hygiene. For the reduction of gingival inflammation, bleeding to probing, reduction of pockets and gain of insertion level the differences were not significant between the groups (p> 0.05). As for bacterial recolonization, both groups behaved similarly for P. gingivalis, T. forsithya and T. denticola.
