Materiales y métodos: Revisión sistemática según lineamientos PRISMA, (enero de 2006 a junio de 2024). Las bases de datos consultadas incluyeron PubMed, Cochrane, Lilacs, ProQuest, OVID y Epistemonikos, mediante Cochrane Risk of Bias 2 y criterios PRISMA se realizó la evaluación, analizando, la frecuencia de Movimientos Intestinales Espontáneos (MIE) y los efectos adversos. Resultados: Se incluyeron 2 ensayos clínicos y 6 revisiones sistemáticas. La lubiprostona incrementó los MIE a 5.1-6.1 por semana frente a 3.8 con placebo (p = 0.046) y en otro estudio, a 5.3 frente a 2.9 con placebo (p < 0.002) esto es 54%. Las náuseas afectaron al 14%-16% de los pacientes tratados con lubiprostona. Los laxantes osmóticos mostraron éxito terapéutico en 52% frente a 11% con placebo (p < 0.001) y redujeron el riesgo de fracaso terapéutico en un 48% (RR = 0.52; IC 95%: 0.46-0.60). Aunque ambos tratamientos demostraron eficacia, los estudios no son comparativos, lo que limita las conclusiones definitivas. Conclusiones: Tanto lubiprostona como laxantes osmóticos son opciones viables para el manejo del ECF en adultos mayores, se requieren estudios comparativos directos para determinar su seguridad y eficacia relativa. Este estudio no recibió financiamiento externo ni fue registrado en PROSPERO.
