Se sabe que la intubación prolongada se asocia a peores resultados, pero otros problemas sistémicos subyacentes, como la sepsis y el traumatismo, también se asocian a este resultado. Asimismo, los pacientes que requieren un manejo avanzado de la vía aérea y una manipulación excesiva tienen más probabilidades de sufrir complicaciones. Existen varios mediadores inflamatorios que se generan durante la intubación orotraqueal, muchos de los cuales pueden considerarse dianas para terapias que ayuden a reducir la inflamación causada por la intubación. Sin embargo, hay pocas pruebas sobre el tratamiento de la respuesta inflamatoria inducida por la intubación orotraqueal en pacientes pediátricos. Por lo tanto, el objetivo de esta revisión narrativa es destacar las complicaciones asociadas a la intubación que pueden surgir de una inflamación mal controlada en pacientes pediátricos intubados, revisar la fisiopatología propuesta detrás de esto y discutir los tratamientos actuales que existen. Por último, teniendo en cuenta la discusión sobre la fisiopatología, describimos las terapias actuales que se están desarrollando y las direcciones futuras que pueden tomarse para crear más opciones de tratamiento en esta población de pacientes. |

It is known that prolonged intubation is associated with worse outcomes, but other underlying systemic issues, such as sepsis and trauma, are also associated with this result. Likewise, patients who require advanced airway management and excessive manipulation are more likely to experience complications. There are various inflammatory mediators that are generated during orotracheal intubation, many of which can be considered targets for therapies to help reduce inflammation caused by intubation. However, there is little evidence on the management of the inflammatory response induced by orotracheal intubation in pediatric patients. Therefore, the aim of this narrative review is to highlight the intubation associated complications that can arise from poorly controlled inflammation in intubated pediatric patients, review the proposed pathophysiology behind this, and discuss the current treatments that exist.
