description Los aneurismas de la aorta abdominal son una condición clínica significativa debido a su alta mortalidad en caso de ruptura. Sin embargo, la eficacia a largo plazo de la terapia endovascular sigue siendo debatida. Este estudio revisa críticamente la evidencia sobre la eficacia y seguridad de cirugía endovascular versus cirugía abierta en aneurismas abdominales rotos. Metodología: Se realizó una revisión sistemática y metaanálisis siguiendo la declaración PRISMA 2020. Se incluyeron ensayos clínicos controlados y aleatorizados que compararon la terapia endovascular y la cirugía abierta en pacientes con aneurisma de aorta abdominal roto. Las bases de datos consultadas incluyeron PubMed, Embase, Web of Science, Scopus, y otras. Se evaluaron los resultados de mortalidad, complicaciones postoperatorias y estancia hospitalaria. Resultados: Se incluyeron cuatro ensayos clínicos con un total de 868 pacientes. No hubo diferencias significativas en la mortalidad a corto plazo (30 días) entre la terapia endovascular y la cirugía abierta. Al año, la mortalidad fue similar entre ambos grupos. A los tres años, la terapia endovascular mostró una ventaja en la supervivencia. No hubo diferencias significativas en las complicaciones a corto plazo entre los dos tipos de tratamiento, la estancia hospitalaria parece ser más corta en el grupo de tratamiento endovascular. Conclusión: La terapia endovascular no difiere en las complicaciones a corto plazo cuando se compara con la cirugía abierta. Las tasas de mortalidad son similares al mes y al año de seguimiento, en el largo plazo la evidencia sugiere una disminución en la mortalidad en el grupo de terapia endovascular. La estancia hospitalaria parece ser menor en el grupo de terapia endovascular.

Resultados: Se incluyeron cuatro ensayos clínicos con un total de 868 pacientes. No hubo diferencias significativas en la mortalidad a corto plazo (30 días) entre la terapia endovascular y la cirugía abierta. Al año, la mortalidad fue similar entre ambos grupos. A los tres años, la terapia endovascular mostró una ventaja en la supervivencia. No hubo diferencias significativas en las complicaciones a corto plazo entre los dos tipos de tratamiento, la estancia hospitalaria parece ser más corta en el grupo de tratamiento endovascular. This study critically reviews the evidence on the efficacy and safety of endovascular surgery versus open surgery in ruptured abdominal aneurysms.
