Introducción: La estenosis traqueal, un estrechamiento permanente de la vía respiratoria, puede derivar en complicaciones graves, incluyendo insuficiencia respiratoria. Diversas causas pueden originarla, como la intubación prolongada, procesos neoplásicos, inmunológicos, infecciosos, traumáticos o idiopáticos. El manejo puede implicar múltiples intervenciones, desde dilataciones por broncoscopia hasta cirugía reconstructiva, dependiendo de la etiología y la gravedad de la estenosis. Metodología El estudio siguió un diseño observacional descriptivo de corte transversal y abordó objetivos específicos: caracterizar sociodemográfica y clínicamente a los pacientes, describir las características de la estenosis traqueal y las complicaciones postoperatorias. La población objetivo incluyó todos los pacientes adultos sometidos a reconstrucción traqueal en el período del 1 de Enero de 2018 al 31 de Diciembre de 2023. Resultados Se identificaron 70 pacientes, 65.71% hombres. La edad promedio fue 44.1 años. El 32.9% tuvo antecedente de infección por covid-19 y el 94.3% antecedente de intubación orotraqueal. Al 71.4% se le realizó reconstrucción traqueal. El 22% de los pacientes operados tuvo complicaciones y hubo mortalidad del 6% Conclusiones La precoz y correcta identificación de síntomas, factores de riesgo, permitirá adecuada orientación diagnostica y enfoque terapéutico temprano en los pacientes con estenosis laringotraqueal. El número de dilataciones y el tiempo que transcurre entre estas y la realización de la reconstrucción traqueal es probable afecte en los desenlaces postoperatorios. Es necesario elaborar mayores estudios prospectivos, para compararlos con la literatura mundial y con nuestro estudio para concluir como la variable COVID19 afecta los desenlaces postoperatorios y la morbimortalidad de esta patología. |

Diversas causas pueden originarla, como la intubación prolongada, procesos neoplásicos, inmunológicos, infecciosos, traumáticos o idiopáticos. El manejo puede implicar múltiples intervenciones, desde dilataciones por broncoscopia hasta cirugía reconstructiva, dependiendo de la etiología y la gravedad de la estenosis. Metodología El estudio siguió un diseño observacional descriptivo de corte transversal y abordó objetivos específicos: caracterizar sociodemográfica y clínicamente a los pacientes, describir las características de la estenosis traqueal y las complicaciones postoperatorias. La población objetivo incluyó todos los pacientes adultos sometidos a reconstrucción traqueal en el período del 1 de Enero de 2018 al 31 de Diciembre de 2023. Resultados Se identificaron 70 pacientes, 65.71% hombres. La edad promedio fue 44.1 años. El 32.9% tuvo antecedente de infección por covid-19 y el 94.3% antecedente de intubación orotraqueal. Al 71.4% se le realizó reconstrucción traqueal. El 22% de los pacientes operados tuvo complicaciones y hubo mortalidad del 6% Conclusiones La precoz y correcta identificación de síntomas, factores de riesgo, permitirá adecuada orientación diagnostica y enfoque terapéutico temprano en los pacientes con estenosis laringotraqueal. El número de dilataciones y el tiempo que transcurre entre estas y la realización de la reconstrucción traqueal es probable afecte en los desenlaces postoperatorios. Es necesario elaborar mayores estudios prospectivos, para compararlos con la literatura mundial y con nuestro estudio para concluir como la variable COVID19 afecta los desenlaces postoperatorios y la morbimortalidad de esta patología.Hospital Santa ClaraEspecialista en Cirugía GeneralEspecializaciónIntroduction: Tracheal stenosis (TS), a permanent narrowing of the airway, can lead to serious complications, including respiratory failure, can be caused by various causes, such as prolonged intubation, neoplastic, immunological, infectious, traumatic or idiopathic processes. TS management may involve multiple interventions, from bronchoscopic dilation to reconstructive surgery, depending on the etiology and severity of stenosis. Methods The study followed a descriptive, observational, cross-sectional design and addressed specific objectives: characterize patients sociodemographically and clinically, describe characteristics of tracheal stenosis and postoperative complications. Target population included all adult patients undergoing tracheal reconstruction from January 1, 2018 to December 31, 2023. Results 70 patients were identified, 65.71% men, average age was 44.1 years. 32.9% had a history of COVID-19 infection and 94.3% had a history of orotracheal intubation. 71.4% underwent tracheal reconstruction. 22% of operated patients had complications and there was a 6% mortality rate. Conclusions The early and correct identification of symptoms and risk factors will allow for adequate diagnostic guidance and early therapeutic approach in patients with laryngotracheal stenosis. The number of dilatations and the time between these and performance of tracheal reconstruction is likely to affect postoperative outcomes.
