En Colombia la Teleodontología (TO) se encuentra regulada a partir del marco legal que le aplica a la telesalud y la telemedicina, sin embargo, su mayor implementación se conoció sin directrices claras durante la pandemia COVID-19. Superada la pandemia, la TO en Colombia se continúa aplicando sin criterios técnicos o lineamientos que encaminen las acciones de prestación de un servicio mediante esta modalidad, seguro y con calidad. Ante la necesidad de implementar mecanismos apropiados para la atención en salud oral de todos los colombianos y que estos sean acordes con el contexto de cada institución prestadora de salud, el objetivo del presente estudio fue construir los lineamientos que deben ser tenidos en cuenta para la implementación de la TO en el servicio de consulta externa con el fin de brindar mayor oportunidad de acceso, continuidad, seguridad en la atención y por ende prevenir la enfermedad y mejorar la salud oral de la población colombiana. Métodos: se realizó un estudio descriptivo con enfoque cualitativo, en el cual se indagaron los requisitos en el marco de estándares de calidad para implementar la atención de TO, mediante la revisión documental de la normatividad vigente del marco legal colombiano y la revisión internacional de la literatura de artículos en las bases de datos Ebsco, Ovied, Medline, PubMed, Proquest, Elsevier y ScienceDirect. En un primer momento se realizó una evaluación de conocimientos y una encuesta de percepción de la implementación del servicio a proponer a 10 odontólogos generales de una IPS que aceptaron participar en el estudio y en un segundo momento se realizó una entrevista a una experta en TO para identificar aquellos aspectos prioritarios para garantizar la prestación del servicio con altos niveles de calidad. Resultados: En relación con los requisitos contemplados en la normatividad se encontró que se deben tener en cuenta para los lineamientos los cinco estándares de habilitación (talento humano, infraestructura, dotación, procesos prioritarios, historia clínica y registros) presentes en la Resolución 3100 de 2019 y Resolución 2654 de 2019. Adicionalmente, se incluyeron 11 artículos y 1 libro en texto completo en donde se identificaron los aspectos y características más importantes de los cinco estándares de habilitación y atributos de calidad para la prestación del servicio. En la evaluación de conocimientos a los odontólogos en relación con el significado, la normatividad y las actividades de TO se evidencia que no tenían conocimiento. En la encuesta de percepción los odontólogos identificaron que la implementación de la TO permite mejorar la oportunidad, accesibilidad, pertinencia, continuidad, seguridad, humanización, capacidad resolutiva, demostrando una buena aceptación de los profesionales. Por último, en la entrevista a la experta se establecieron aspectos relevantes de lo indispensable y las recomendaciones para el diseño de los lineamientos a proponer. Conclusión: La implementación de la TO no solo representa una mejora en el acceso y la eficiencia del servicio, sino que también es una herramienta poderosa para garantizar la calidad y la continuidad de la atención en salud oral. Teniendo en cuenta los resultados, se diseñaron los lineamientos para la implementación del servicio de TO planteando así mismo recomendaciones para su implementación, seguimiento y control. |

Superada la pandemia, la TO en Colombia se continúa aplicando sin criterios técnicos o lineamientos que encaminen las acciones de prestación de un servicio mediante esta modalidad, seguro y con calidad. Ante la necesidad de implementar mecanismos apropiados para la atención en salud oral de todos los colombianos y que estos sean acordes con el contexto de cada institución prestadora de salud, el objetivo del presente estudio fue construir los lineamientos que deben ser tenidos en cuenta para la implementación de la TO en el servicio de consulta externa con el fin de brindar mayor oportunidad de acceso, continuidad, seguridad en la atención y por ende prevenir la enfermedad y mejorar la salud oral de la población colombiana. Métodos: se realizó un estudio descriptivo con enfoque cualitativo, en el cual se indagaron los requisitos en el marco de estándares de calidad para implementar la atención de TO, mediante la revisión documental de la normatividad vigente del marco legal colombiano y la revisión internacional de la literatura de artículos en las bases de datos Ebsco, Ovied, Medline, PubMed, Proquest, Elsevier y ScienceDirect. En un primer momento se realizó una evaluación de conocimientos y una encuesta de percepción de la implementación del servicio a proponer a 10 odontólogos generales de una IPS que aceptaron participar en el estudio y en un segundo momento se realizó una entrevista a una experta en TO para identificar aquellos aspectos prioritarios para garantizar la prestación del servicio con altos niveles de calidad. Resultados: En relación con los requisitos contemplados en la normatividad se encontró que se deben tener en cuenta para los lineamientos los cinco estándares de habilitación (talento humano, infraestructura, dotación, procesos prioritarios, historia clínica y registros) presentes en la Resolución 3100 de 2019 y Resolución 2654 de 2019. Adicionalmente, se incluyeron 11 artículos y 1 libro en texto completo en donde se identificaron los aspectos y características más importantes de los cinco estándares de habilitación y atributos de calidad para la prestación del servicio. En la evaluación de conocimientos a los odontólogos en relación con el significado, la normatividad y las actividades de TO se evidencia que no tenían conocimiento. En la encuesta de percepción los odontólogos identificaron que la implementación de la TO permite mejorar la oportunidad, accesibilidad, pertinencia, continuidad, seguridad, humanización, capacidad resolutiva, demostrando una buena aceptación de los profesionales. Por último, en la entrevista a la experta se establecieron aspectos relevantes de lo indispensable y las recomendaciones para el diseño de los lineamientos a proponer. Conclusión: La implementación de la TO no solo representa una mejora en el acceso y la eficiencia del servicio, sino que también es una herramienta poderosa para garantizar la calidad y la continuidad de la atención en salud oral. After the pandemic, teledentistry in Colombia continues to be applied without technical criteria or guidelines that direct the actions to provide a safe and quality service through this modality. Given the need to implement appropriate mechanisms for oral health care for all Colombians and that these are consistent with the context of each health care institution, the objective of this study was to build the guidelines that must be taken into account for the implementation of teledentistry in the outpatient service in order to provide greater opportunity for access, continuity, safety in care and therefore prevent disease and improve the oral health of the Colombian population. Methods: A descriptive qualitative study was conducted, in which the requirements within the framework of quality standards for implementing teledentistry care were investigated, through a documentary review of the current regulations of the Colombian legal framework and an international literature review of articles in the Ebsco, Ovied, Medline, PubMed, ProQuest, Elsevier and ScienceDirect databases. First, a knowledge assessment and a perception survey of the implementation of the service to be proposed were conducted with 10 general dentists from an IPS who agreed to participate in the study. Second, an interview was conducted with a teledentistry expert to identify those priority aspects to guarantee the provision of the service with high levels of quality. Results: Regarding the requirements contemplated in the regulations, it was found that the five enabling standards (human talent, infrastructure, staffing, priority processes, clinical history and records) present in Resolution 3100 of 2019 and Resolution 2654 of 2019 must be taken into account for the guidelines. Additionally, 11 articles and 1 full-text book were included in which the most important aspects and characteristics of the five enabling standards and quality attributes for the provision of the service were identified. In the evaluation of the knowledge of dentists in relation to the meaning, regulations and activities of teledentistry, it is evident that they had no knowledge. In the perception survey, dentists identified that the implementation of teledentistry allows improving opportunity, accessibility, relevance, continuity, security, humanization, and problem-solving capacity, demonstrating good acceptance by professionals. Finally, in the expert interview, the relevant aspects of what is essential and the recommendations for the design of the guidelines to be proposed were established. Conclusion: The implementation of teledentistry not only represents an improvement in access and efficiency of the service, but is also a powerful tool to ensure the quality and continuity of oral health care.
