Las posibilidades de detectar cardiopatías congénitas mejoran siguiendo protocolos estructurados. La cardiopatía fetal es una de las principales malformaciones congénitas graves. El objetivo de esta revisión es presentar los beneficios de la evaluación cardiaca secuencial y rutinaria en el primer y segundo trimestre e identificar las estructuras y patologías que pueden detectarse a esta edad gestacional. Se buscaron en las bases de datos PubMed, Medline, MD consult, Embase, Clinical Key, Scielo y ScienceDirect, así como en textos especializados en español e inglés, estudios diagnósticos, revisiones sistemáticas y metaanálisis relacionados con los términos \early fetal echocardiography" y \congenital heart malformations" publicados entre 2000 y 2019. Los avances tecnológicos han revolucionado el examen ecocardiográfico fetal haciendo posible el diagnóstico de cardiopatías congénitas desde el primer y segundo trimestre del embarazo. Sin embargo, hay que reconocer que la ecocardiografía fetal precoz tiene limitaciones como la evaluación de venas pulmonares y lesiones cardiacas que son progresivas. El beneficio encontrado fue la detección precoz de patologías con alta sensibilidad y especificidad. Se incluyó una secuencia de evaluación cardiaca fetal en estos exámenes ecocardiográficos fetales tempranos debido al conocimiento de las diferencias anatómicas y hemodinámicas fetales en el primer trimestre y el comienzo del segundo trimestre. La ecocardiografía fetal precoz es una herramienta muy útil para la detección de cardiopatías congénitas; permite informar precozmente a los padres sobre el pronóstico y los posibles tratamientos. La exploración cardiaca fetal en el primer trimestre es segura y puede detectar la mayoría de las malformaciones cardiacas congénitas. |

Fetal heart disease is one of the main serious congenital malformations. The objective of this review is to present the bene ts of sequential and routine cardiac evaluation in the rst and early second trimesters and to identify the structures and pathologies that can be detected at this gestational age. The databases of PubMed, Medline, MD consult, Embase, Clinical Key, Scielo, and ScienceDirect, as well as specialized texts in Spanish and English were searched for diagnostic studies, systematic reviews, and meta-analysis related to the terms \early fetal echocardiography" and \congenital heart malformations" published between 2000 and 2019. Technological advances have revolutionized the fetal echocardiographic examination making possible the diagnosis of congenital heart disease from the rst and early second trimester of pregnancy. However, it should be recognized that early fetal echocardiography has limitations such as the evaluation of pulmonary veins and cardiac lesions that are progressive. The bene t found was earlier detection of pathologies with high sensitivity and speci city. A fetal cardiac evaluation sequence was included in these early fetal echocardiographic examinations due to the awareness of fetal anatomical and hemodynamic di erences in the rst trimester and the beginning of the second trimester. Early fetal echocardiography is a very useful tool for the detection of congenital heart disease; it allows parents to be informed about the prognosis and possible treatments early.
