description Objectives: To analyze the problems of the focalization policy and the characteristics of decisionmaking to assign health subsidies for the population of Sisben levels 3 and 4 of 4 localities in Bogotá, in order to propose criteria for transforming the processes of policy formulation and correct its defects on marginalization and exclusion. Methods: A critical, participative and pluralist methodological approach, which combined multiple research methods and techniques from a systemic and post-estructuralist perspective, was undertaken. For sub-projects were carried out. The subsidy-focalization policy in Colombia was analyzed based unpon the arguments of the main socio-political actors who participated in its formulation. In order to establish how the focalization policy of health spending is perceived by the excluded population, the public providers and the local authorities, a qualitative study was done based upon focal groups and in-depth interviews. A follow-up of the accessibility to and use of health services was also carried out from the definition of a tracing condition. Finally, collective  recommendations of policies were formulated from the perspective of the excluded population, the providers and the authorities. Results: The problems of a focalization policy, product of the adoption of a narrow and utilitarian concept of poverty and social justice, of a scarcely participative process of analysis and measurement of poverty, of the screening and setup of the programs and of the evaluation of the results of the policy, are identified. Conclusions: Measures to facilitate and make health care accessibility more equalitarian are justified not only because of poverty, but also because of the correction of inequities in health which result from other determinants that can deepen inequalities, marginalization and exclusion. The adoption of a broad approach for analyzing poverty, for decision-making and for social participation is proposed.

The adoption of a broad approach for analyzing poverty, for decision-making and for social participation is proposed.Objetivos: Analizar los problemas de la política de focalización y las características de la toma de decisiones para asignar subsidios en salud a la población de los niveles 3 y 4 del Sisben de cuatro localidades de Bogotá, buscando aportar elementos de juicio para transformar los procesos de formulación de políticas y corregir sus defectos en materia de marginalización y exclusión. Métodos: Se asumió un enfoque metodológico crítico, participativo y pluralista que combinó múltiples métodos y técnicas de investigación desde una perspectiva sistémica y posestructuralista. Se realizaron cuatro subproyectos. Se analizó la política de focalización de subsidios en Colombia con base en el análisis del discurso de los principales actores sociopolíticos que participaron en su formulación. Se realizó un estudio cualitativo para establecer cómo es percibida la política de focalización del gasto en salud por la población excluida, por los prestadores públicos y las autoridades locales, con base en grupos focales y entrevistas a profundidad. Se hizo también un seguimiento del acceso y uso de servicios médicos a partir de la definición de una condición trazadora. Finalmente, se formularon  recomendaciones colectivas de política desde la perspectiva de la población excluida, de los prestadores y de las autoridades. Resultados: Se problematiza la política de focalización producto de la adopción de un concepto estrecho y utilitarista de la pobreza y de la justicia social, del carácter poco participativo del proceso de análisis y medición de la pobreza, del monitoreo y la puesta en práctica de los programas, y de la evaluación de los  resultados de la política. Conclusiones: Las medidas para facilitar y hacer más equitativo el acceso a los servicios de salud no se justifican sólo en virtud de la pobreza, sino de la corrección de las inequidades en salud que resultan de otros determinantes que pueden agravar la desigualdad, la marginación y la exclusión.
