Método Cohorte retrospectiva de datos administrativos de pacientes adultos inscritos en el sistema contributivo de salud de Colombia que se sometieron a artroplastia de hombro entre los años 2012 y 2017. Los desenlaces evaluados fueron tasa de mortalidad, ingreso a UCI y requerimiento de trasfusión durante los primeros 30 días, tasa de mortalidad, fracturas, infección y revisiones a 1 año y el costo total del episodio médico incurrido por el tercer pagador durante los 90 días posteriores al procedimiento. También se generaron modelos de regresión lineal multinivel para determinar los factores pronósticos asociados con los resultados presentados. Resultados El estudio incluyó a 2.471 pacientes, el grupo de edad entre 65 y 79 años representa el 55% de la totalidad, siendo más frecuente en el sexo femenino. las tasas complicaciones dentro de los primeros 30 días por cada 100 procedimientos fue de 0.36 para mortalidad, 2.14 para ingreso a UCI y 0.57 para requerimiento transfusional. las tasas complicaciones a un año por cada 100 procedimientos fue de 2.67 para mortalidad, 0.97 para revisiones e infecciones peri protésicas y 0.89 para fracturas peri protésicas. Se encontró que a mayor edad e índice de comorbilidad de Charlson (ICC) se incrementa la tasa de mortalidad; menor edad y el sexo masculino están asociados con mayor tasa de revisión al año. Se encontró que el costo total promedio del episodio médico fue de USD$2031.12. El modelo multivariado encontró tendencias crecientes en el costo a medida que aumenta el ICC. Conclusiones Se trata de un estudio poblacional que incluye la cohorte estudiada para mortalidad en reemplazo de hombro más extensa a las publicadas hasta el momento. El comportamiento en cuanto mortalidad es mayor en Colombia, se reportan menores costos desde el punto de vista del sistema de salud. La edad y el ICC son factores asociados con mortalidad. Method Retrospective cohort of administrative data of adult patients enrolled in the Colombia’s contributory health system who underwent shoulder arthroplasty between 2012 and 2017. The outcomes evaluated were mortality, ICU admission, and transfusion requirement rate during the first 30 days, mortality, fractures, infection, and revisions rate at 1 year and the total cost of the medical event incurred by the third payer during the 90 days after the procedure. Multilevel linear regression models were also generated to determine the prognostic factors associated with the results presented. Results 2,471 patients were included, the age group between 65 and 79 years represents 55% of these, being more frequent in the female sex. Complication rates within the first 30 days per 100 procedures were 0.36 for mortality, 2.14 for ICU admission, and 0.57 for transfusion requirements. Complication rates at one year per 100 procedures were 2.67 for mortality, 0.97 for revisions and periprosthetic infections, and 0.89 for periprosthetic fractures. It was found that the higher the age and the Charlson comorbidity index (CCI), the mortality rate increases; Younger age and male gender are associated with a higher revision rate per year. The average total cost of the medical episode was found to be USD$2031.12. The multivariate model found increasing trends in cost as the ICC increases. Conclusions This is a population-based study that includes the largest cohort studied for mortality in shoulder replacement to those published to date in Colombia and developing countries. The behavior in terms of mortality is higher in Colombia, lower costs are reported from the point of view of the health system.
