Whether the rationality of health insurance is compatible with the rationality of cost-effective public health activities is questioned as well as if the health plans designed under the latter vision insure the individual people or the state. The question of whether the basic health and mother-child plans, considered nowadays as health insurance in several Latin American countries and promoted by multilateral agencies, are not different from the same public health plans which have always existed and if therefore the term social security is simply being misused and abused of, is examined.Se analiza la actual forma de medir la cobertura de la seguridad social en Colombia y Latinoamérica, sin tener en cuenta los principios básicos de la seguridad social. Se cuestiona si la racionalidad del aseguramiento es compatible con la racionalidad de las actividades costo efectivas de salud pública y, si los planes de salud construidos bajo la segunda óptica aseguran a los individuos o al Estado.
