Objetivos: Describir los efectos del ayuno en el estado hídrico, gasto urinario y volúmenes plasmático y sanguíneo; evidenciados por cambios en el peso, hematocrito y volumen urinario. Métodos: Se realizó un estudio prospectivo cuasiexperimental sin grupo control; se llevaron 18 sujetos (10 hombres, 8 mujeres) a un punto de hipohidratación leve por medio de ayuno absoluto de sólidos y líquidos en un periodo máximo de 24 horas; se midió el peso y el gasto urinario de manera seriada y se tomaron muestras sanguíneas al comienzo y al final del periodo de ayuno. Resultados: Se encontró un déficit hídrico de 16 ml/Kg tras 12 horas de ayuno y una pérdida de 1,038 g/Kg/h en adelante. El tiempo medio para llegar a la hipohidratación fue de 15:09 horas (±3,3h); los sujetos presentaron un descenso del gasto urinario a un promedio de 0,6 ml/Kg/h y se calculó una reducción de los volúmenes sanguíneo y plasmático de 2,9% y 4,18% respectivamente. Conclusiones: El ayuno prolongado como aquel que se presenta antes de un procedimiento quirúrgico puede provocar hipohidratación y oliguria. Esto podría afectar la respuesta ante el reto anestésico y quirúrgico; aumentando la susceptibilidad de complicaciones renales, hemodinámicas, entre otras.
