En cavidad orofaringe se describen dos tipos: aquellos relacionados con la infección del virus del papiloma humano (VPH) relacionado con la variante histológica basaloide y aquellos que no, los cuales a su vez se relacionan con otros factores de riesgo como el uso de tabaco y el consumo de alcohol, además de la variante histológica convencional . El carcinoma relacionado con VPH usualmente tiene un mejor pronóstico incluso en etapas avanzadas debido a que inicialmente no posee la mutación para el gen TP53 a diferencia del no relacionado con VPH que la tiene. Materiales y métodos El objetivo de este proyecto es encontrar las diferencias en la expresión por inmunohistoquímica de un marcador subrogado para la infección por VPH (p16) y de la expresión de p53, utilizando escalas aceptadas, así como su asociación con factores de riesgo y variables histológicas, en pacientes con carcinoma escamocelular de cavidad oral y orofaringe del Hospital Universitario Nacional entre los años 2017-2021 . Resultados Se encontraron 50 pacientes con carcinoma escamocelular (18 [36%] mujeres, 32[64%] hombres) con una edad promedio de 64 años, en 27 (54%) cavidad oral con una prevalencia del variante convencional de 18(66%) y 23 (46%) en orofaringe con 12 (52%) de la variante basaloide. Para la marcación con inmunohistoquímica el 19 (38%) fueron positivos para p16 en orofarínge y 21 (40%) positivos para p53 en cavidad oral, la distribución para factores de riesgo fue en p16, el 1 (5%) con consumo de alcohol únicamente, 3 (15%) uso exclusivo de tabaco y 4 (20%) uso de ambos; para p53 3 (30%) pacientes con uso de tabaco exclusivamente, 3 (30%) con consumo de alcohol exclusivamente y 4 (40%) con uso de ambos Conclusión La asociación con infección por VPH es más común en orofaringe con variante basaloide, el consumo de alcohol y tabaco es más común en cavidad oral, con variante histológica convencional, por otra parte, la marcación para p16 acorde a lo esperado, sin embargo, la marcación para p53 fue heterogénea con algunos datos inesperados como marcación doble (p16 y p53). Two types are described in the oropharynx cavity: those related to human papillomavirus (HPV) infection related to the basaloid histological variant and those that are not, which in turn are related to other risk factors such as tobacco use and alcohol consumption, and usually present with a conventional histological variant. HPV-related carcinoma usually has a better prognosis even in advanced stages because it does not initially have the TP53 gene mutation, unlike non-HPV-related carcinoma that does. Materials and methods The objective of this project is to find the differences in the expression by immunohistochemistry of a surrogate marker for HPV infection (p16) and the expression of p53, using accepted scales, as well as their association with risk factors and histological variants, in patients with squamous cell carcinoma of the oral cavity and oropharynx at the Hospital Universitario Nacional between the years 2017-2021. Results Fifty patients with squamous cell carcinoma (18 [36%] women, 32 [64%] men) with a mean age of 64 years were found. 27 (54%) in oral cavity with a prevalence of the conventional variant of 18 (66%). 23 (46%) in the oropharynx with 12 (52%) of the basaloid variant. For immunohistochemical staining, 19 (38%) were positive for p16 in the oropharynx and 21 (40%) positive for p53 in the oral cavity, the distribution for risk factors was in p16, 1 (5%) with only alcohol consumption, 3 (15%) exclusive use of tobacco and 4 (20%) use of both; for p53, 3 (30%) patients used tobacco exclusively, 3 (30%) used alcohol exclusively, and 4 (40%) used both. Conclusion The association with HPV infection is more common in the oropharynx with a basaloid variant, alcohol and tobacco consumption is more common in the oral cavity, with a conventional histological variant.
