Métodos: Se incluyeron casos con diagnóstico histopatológico de carcinoma de tiroides entre los años 2017 y 2020, cuyos reportes de patología fueron revisados para recopilar información respecto a las variables sociodemográficas y patológicas asociadas con recurrencia. Se registraron los datos y se cuantificaron medidas de frecuencia para la muestra en general y con las submuestras: microcarcinomas y tumores mayores de un 1 centímetro. Resultados: De los 959 casos, se presentaron con mayor frecuencia en mujeres (84.98%) y en edades entre los 40 a 59 años (46.71%), el tipo histológico más frecuente fue el papilar (95.93%), seguido por el medular (1.15%). El 40.46% de los casos correspondían a microcarcinomas, los cuales presentaban menor frecuencia de características asociadas a recurrencia en comparación con los tumores mayores de un centímetro: metástasis a ganglios linfáticos (38.15% vs 63.15%), extensión extratiroidea macroscópica (1.80% vs 8.95%), compromiso de los márgenes de sección (20.10% vs 44.04%). Dentro de los microcarcinomas con metástasis a ganglios linfáticos hay una mayor presentación a edades más tempranas y de tumores multifocales, en comparación con los que no presentan metástasis. Conclusiones: El carcinoma de tiroides es una entidad frecuente en Colombia, donde los microcarcinomas comprenden una importante proporción de los casos, y los cuales se caracterizan por presentar un comportamiento menos agresivo dado por una menor prevalencia de características asociadas a recurrencia, en comparación con tumores mayores de 1 centímetro.Objectives: In a case series with diagnosis of thyroid cancer, to make a description about the clinical and pathological features, especially those which are related to biological behavior and recurrence risk. Methods: A search was performed in the archives of the pathology´s laboratory at Hospital Universitario Nacional, the cases with pathological diagnosis of thyroid carcinoma between the years of 2017 and 2020 were selected. Each report of pathology was reviewed, and the information related to sociodemográfic and pathological features were recorded in a database, and statistical measures like percentage and absolute frequencies were calculated. The results were compared against researchs performed in other countries. Results: 959 cases of thyroid carcinoma were found, 84.98% were in female patients, the most common age was between 40 and 59 years (46.71%). The papillary carcinoma was the most common histologic type (95.93%), followed by medullary carcinoma (1.15%), 40.46% were microcarcinomas, which showed less prevalence versus larger carcinomas in variables such as: lymph node metastasis (38.15% vs 63.15%), gross extrathyroidal invasion (1.80% vs 8.95%) and positive surgical margins (20.10% vs 44.04%). The microcarcinomas with lymph node metastasis tends to occur frequently in younger people and when there are multifocal.
