Objetivo: Determinar los efectos del ejercicio sobre la vía de señalización de HIF-1α, la función cardiopulmonar y la calidad de vida en pacientes con EPOC residentes a gran altitud. Métodos: Pacientes con EPOC físicamente inactivas residentes en gran altitud, comparados con sujetos control sanos. Intervención: programa de ejercicio físico domiciliar multimodal de 8 semanas (8-WVP) compuesto por ejercicio respiratorios, resistencia cardiopulmonar, fuerza resistencia, flexibilidad y sesiones de educación en salud. Variables primarias: concentración plasmática de HIF-1α, EPO y VEGF en reposo, 30 minutos y 24 horas después de ergoespirometría. Variables secundarias: espirometría, hemograma, rendimiento físico, test funcionales de aptitud física y calidad de vida asociada a la salud. Resultados: Los niveles de HIF-1α fueron más altos en los pacientes EPOC que en los controles. El ejercicio disminuyó los niveles de HIF-1α en 25 %, aumentó el nivel de VEGF en 34 % y la EPO en 23 % en pacientes con EPOC. Existe una correlación negativa entre HIF-1α y el V̇O2pico, VE, CVF, FEF 25-75%, MCV y MCH. El rendimiento físico de los EPOC aumentó en todas las pruebas de aptitud física. Conclusión: Los niveles de HIF-1α en los EPOC fueron significativamente más altos que en personas sanas antes del ejercicio. Reposo (R), 1 hora antes de la CPET, 2. Ejercicio (E), 30 minutos después de finalizar la CPET, 3. Post-ejercicio (P), 24 horas después de CPET.
