Es así como la guía NICE 2016 se convierte en una excelente herramienta al contar con escalas de clasificación y estudios con los que estamos familiarizados y fácilmente están disponibles. This is how the NICE 2016 guide becomes an excellent tool by having classification scales and studies that we are familiar with and are easily available.Especialidades MédicasAnestesiologoSe hizo una evaluación económica sobre los costos en los que se incurre en la solicitud de paraclínicos preoperatorios, y a través de esta se realizó un análisis retrospectivo de la base de datos de los registros de la consulta de valoración preanestésica del Hospital Universitario Nacional de Colombia, en el período comprendido entre abril de 2019 y marzo de 2020; incluyendo en el análisis una muestra de 6 registros por mes, para un total de 66 registros durante el tiempo del estudio. Para su realización fue necesaria la aprobación del comité de ética de la Universidad Nacional de Colombia; y no requirió firma de consentimiento informado dado que fue clasificada con una investigación de riesgo mínimo al tratar información de manera retrospectiva sin intervención ni modificación en las variables biológicas, fisiológicas, psicológicas o sociales del individuo, sin exponerlos a riesgos derivados de la investigación. Los criterios de inclusión fueron pertenecer a los registros incluidos en la base de datos de valoración preanestésica del Hospital Universitario Nacional de Colombia en el período del estudio, corresponder a un individuo mayor de 18 años y estar programado para cirugía no cardíaca. Fueron excluidos los registros pertenecientes a individuos gestantes o que se encontraran duplicados. La selección de registros se hizo de manera aleatoria en secuencia y por computador, sin cegamiento. Se tomaron datos relacionados con la especialidad quirúrgica tratante, procedimiento a realizar, valoración clínica del paciente y los estudios prequirúrgicos solicitados. La información obtenida de la base de datos fue convertida en registros anónimos, y se eliminaron todos los datos que guardaban relación con la identificación de los pacientes en cada uno de los registros. Se describieron variables para definir el exceso de costo promedio por paciente y el exceso de costo por grupo, y se estableció el exceso como el conteo de los paraclínicos solicitados por encima del parámetro establecido en la guía, y el defecto como el conteo de los paraclínicos solicitados por debajo del parámetro establecido en la guía; definiendo los límites inferior y superior en un (1) paraclínico, correlacionándolo con el costo monetario final individual y total de estos. El procesamiento de esta información permite establecer si existe o no una diferencia de costos al adherirse a las guías NICE 2016, cuando en el análisis primario del exceso de costo promedio por paciente tiene un valor de p<0.05. El responsable de la revisión de las historias clínicas, el manejo y procesamiento de la información, fueron el investigador principal y el asesor metodológico a través del software Microsoft Excel para Office 365©, para uso educativo. Todo registro y procesamiento de la información se hizo de manera electrónica y no se utilizaron formatos de papel. Los datos de los registros fueron procesados mediante estadística descriptiva, haciendo una inferencia de costos por paciente reflejado en medidas de dispersión. El análisis de las variables se hizo según se clasifica cada una, así, continuas mediante promedios y desviaciones estándar, las variables discretas mediante medianas y rangos intercuartílicos, y las variables dicotómicas nominales y ordinales con proporciones. La comparación del exceso de costo individual se hará a través de T de student, y las variables dicotómicas mediante Chi cuadrado. Un valor de p<0.05 fue considerado significativo para rechazar la hipótesis nula. El análisis por subgrupos fue realizado según especialidades quirúrgicas, estado de saludo previo del paciente medido con la escala ASA, grupos de edad, riesgo quirúrgico medido con la escala de John Hopkins, entre otros. En relación con las consideraciones éticas de este trabajo, se tuvieron en cuenta los principios de la resolución 008430 de 1993 (22), dado que el presente estudio se trata de una investigación dirigida al estudio de las técnicas y métodos que se recomiendan para la prestación de servicios de salud, con resultados que no pueden ser obtenidos por otro medio. Dado que no es un estudio que trate el desarrollo de fármacos o dispositivos biomédicos no corresponde tener certificado de buenas prácticas clínicas a ninguno de los autores, ya que hace su objeto de investigación eventos de farmacoeconomía.
