Métodos: Análisis descriptivo y retrospectivo de los ensayos clínicos realizados en 19 países de América Latina y el caribe latino en las plataformas ICTRP y Clinicaltrials.gov. Se incluyó una descripción del marco regulatorio para la ejecución y transparencia y la selección de estudios de caso para analizar la relevancia estas investigaciones en la toma de decisiones. Resultados: Un total de 8516 registros de ensayos clínicos fueron incluidos en el análisis. Predominan los ensayos clínicos fase 3 patrocinados por la industria farmacéutica para cáncer. La transparencia de los ensayos clínicos presenta importantes avances en cuanto al registro previo de los ensayos clínicos. Respecto al marco regulatorio vigente, los reglamentos para la ejecución de la investigación clínica tienen como base las BPC. Sin embargo, la regulación de la transparencia sobre la investigación clínica y el uso de sus resultados para la toma de decisiones todavía es incipiente. Conclusiones: Las presiones globales han alejado a América Latina del desarrollo de investigaciones clínicas que respondan a sus propias necesidades y vacíos de información. La transparencia requiere inversión, respaldada por voluntad política para mejorar la confianza, las oportunidades de cooperación internacional y proteger a la población. La opacidad de la información dificultó realizar una mejor interpretación sobre la utilidad de los ensayos clínicos para la toma de decisiones regulatorias. (Texto tomado de la fuente)Objective: To describe and characterize clinical research of medicines in Latin American countries for the periods 2011 to 2021 from global and local databases. Methods: This is a descriptive and retrospective analysis of clinical trials conducted in 19 countries in Latin America and the Latin Caribbean on the ICTRP and Clinicaltrials.gov platforms. A description of the regulatory framework for development and transparency and the selection of case studies were included to assess the relevance of these investigations in decision-making. Results: A total of 8516 clinical trial records were included in the analysis. Phase 3 clinical trials sponsored by the pharmaceutical industry for cancer predominate. The transparency of clinical trials presents important advances in terms of the prior registration of clinical trials. Regarding the current regulatory framework, the regulations for the execution of clinical research are based on GCP. However, the regulation of transparency about clinical research and the use of its results for decision-making is still in its infancy. Conclusions: Global pressures have distanced Latin America from the development of clinical research that responds to its own needs and information gaps. Transparency requires investment and political will to improve trust, opportunities for international cooperation and protect the population.
