Materiales y métodos: El grupo desarrollador contó con la participación de expertos temáticos en Microbiología, Salud pública, Medicina interna, Infectología, Obstetricia, Medicina materno fetal e Infectología gineco-obstétrica y perinatal. También hicieron parte de este grupo profesionales con entrenamiento en epidemiología clínica y en búsqueda sistemática de la información. El grupo también contó con la participación de representantes de la secretaría de salud y de la Asociación Bogotana de Obstetricia y Ginecología. Los participantes presentaron sus conflictos de interés. A partir de una pregunta clínica se realizó la graduación de los desenlaces y se realizó una búsqueda sistemática que abarcó las bases de datos PubMed, Embase, Lilacs, Bireme. La pesquisa se amplió a repositorios institucionales y reportes de vigilancia de resistencia antimicrobiana, sin restricción de idioma o fecha, la búsqueda se actualizó el 1 de octubre de 2022. Se utilizó la metodología GRADE (Grading of Recommendations Assessment, Development and Evaluation) para valorar la calidad de la evidencia y establecer la fuerza de las recomendaciones. Finalmente se utilizó la metodología RAND/UCLA para el consenso formal. Este documento fue revisado por pares académicos previo a su publicación. Resultados: Se muestran las recomendaciones y puntos de buena práctica clínica del consenso para el tratamiento de la infección de vías urinarias altas durante la gestación, así como la evidencia que soporta cada una de ellas. Se recomienda que el manejo inicial de la paciente debe ser intrahospitalario y el tratamiento antimicrobiano empírico de primera opción sea con cefalosporinas de segunda generación. Se sugiere que la segunda opción, en segundo y tercer trimestre sea con aminoglucósidos y que la tercera opción sea con cefalosporinas de tercera generación. En gestantes con antecedente de infección por microorganismos con resistencia a cefalosporinas de tercera o cuarta generación se recomienda como primera opción el uso de carbapenémicos, como segunda opción el uso de aminoglucósidos o cefalosporinas de cuarta generación y como tercera opción el uso de piperacilina/tazobactam. Se recomienda modificar la antibioticoterapia guiada por el resultado del antibiograma. Conclusiones: Dentro de los puntos clave del consenso se resaltan los esquemas terapéuticos sugeridos al igual que la duración de la terapia. (Texto tomado de la fuente)Objective: To generate evidence-informed recommendations, through a formal consensus, aimed at the treatment of upper urinary tract infection during pregnancy. Materials and methods: The development group had the participation of thematic experts in Microbiology, Public Health, Internal Medicine, Infectology, Obstetrics, Maternal-Fetal Medicine and Gynecological-Obstetric and Perinatal Infectology. Professionals with training in clinical epidemiology and systematic search for information were also part of this group. The group was also attended by representatives of the Ministry of Health and the “Asociación Bogotana de Obstetricia y Ginecología”. Participants presented their conflicts of interest. Based on a clinical question, the graduation of the outcomes was performed and a systematic search was carried out that covered the databases PubMed, Embase, Lilacs, Bireme. The research was extended to institutional repositories and antimicrobial resistance surveillance reports, with no language or date restriction, the search was updated on 1 October 2022. We used the GRADE (Grading of Recommendations Assessment, Development and Evaluation) methodology to assess the quality of the evidence and establish the strength of the recommendations. Finally, the RAND/UCLA methodology was used for formal consensus. This paper was peer-reviewed prior to publication. Results: The recommendations and points of good clinical practice of the consensus for the treatment of upper urinary tract infection during pregnancy are shown, as well as the evidence that supports each of them. It is recommended that the initial management of the patient should be in-hospital and the empirical antimicrobial treatment of first choice is second-generation cephalosporins. It is suggested that the second option, in the second and third trimester be with aminoglycosides and that the third option be with thirdgeneration cephalosporins. In pregnant women with a history of infection by microorganisms with resistance to third or fourth generation cephalosporins, the use of carbapenems is recommended as the first option, the use of aminoglycosides or fourthgeneration cephalosporins as a second option and the use of piperacillin/tazobactam as a third option. It is recommended to modify the antibiotic therapy guided by the result of the antibiogram.
