La fístula enteroatmosférica (FEA) es una variante que se comunica con una herida abdominal abierta. Con respecto a las causas, las fistulas iatrogénicas, estas fístulas pueden surgir por cirugía abdominal. La morbilidad y mortalidad son elevadas, con un 90% de pacientes experimentando morbilidad y tasas de mortalidad del 5-20%, siendo la sepsis la principal causa de muerte. El manejo se enfoca en cuatro fases: diagnóstico y estabilización, plan nutricional y manejo de la herida, evaluación radiológica para definir la anatomía, y cierre definitivo en casos no espontáneos. El costo hospitalario es alto, superando los 500,000 USD en Estados Unidos, requiriendo un equipo multidisciplinario. En el Hospital Universitario Nacional (HUN), los pacientes con FEC y FEA son motivo frecuente de consulta, con numerosos ingresos hospitalarios y procedimientos. Este Estándar Clínico Basado en la Evidencia (ECBE) tiene como objetivo proporcionar recomendaciones para el diagnóstico, tratamiento y seguimiento de pacientes con FEC o FEA en el HUN, optimizando la calidad de atención y la calidad de vida mediante un enfoque interdisciplinario basado en la mejor evidencia disponible. El ECBE se desarrolló mediante un proceso secuencial de siete fases: i) conformación del grupo desarrollador; ii) definición del alcance y los objetivos del ECBE; iii) revisión sistemática de guías de práctica clínica (GPC); iv) elaboración del algoritmo preliminar; v) desarrollo de un acuerdo interdisciplinario; vi) elaboración del algoritmo final, y vii) revisión y edición del ECBE El manejo integral de paciente con FEC y FEA en el HUN, guiada por un ECBE, representa un paso significativo hacia la optimización de la calidad de atención. The enterocutaneous fistula with exposure to the atmosphere (EAF) is a variant that communicates with an open abdominal wound. As for the cause of iatrogenic fistulas, these fistulas can arise because of abdominal surgery. Morbidity and mortality rates are high, with 90% of patients experiencing morbidity and mortality rates ranging from 5-20%, with sepsis being the leading cause of death. Management focus is broken down into four phases: diagnosis and stabilization, nutritional planning and wound management, radiological assessment to define the anatomy, and definitive closure (in non-spontaneous cases). The management of fistulas requires a multidisciplinary team and incurs a cost exceeding 500,000 USD in the US. At the National University Hospital (HUN), patients with ECF and EAF are a frequent reason for consultation leading to numerous hospital admissions and procedures. This Evidence-Based Clinical Standard (ECBE) aims to provide recommendations for the diagnosis, treatment, and follow-up of patients with ECF or EAF at HUN, optimizing the quality of care and quality of life through an interdisciplinary approach based on the best available evidence. The ECBE was developed via a sequential process of seven phases: i) formation of the developer group; ii) definition of the scope and objectives of the ECBE; iii) systematic review of clinical practice guidelines (CPGs); iv) development of the preliminary algorithm; v) establishment of interdisciplinary agreement; vi) development of the final algorithm, and vii) review and editing of the ECBE.
