Summary:ABSTRACT: Decentralization of the health sector entails fiscal, political, social and administrative changes in order that municipalities assume functions transferred by the central and departmental governments through which government becomes responsible for health of people. This study analyses the characteristics of institutional development and community participation as main aspects of decentralization. The findings show that in spite of the fact that the three municipalities studied (Itagüí, La Ceja and Balboa) have different levels of social and economic development, they have similar advances in implementing decentralization and have faced many obstacles such as the lack of political will of mayors to support the process, the lack of technical capacity of the staff of local health head quarters and their constant changes as a consequence of political lobbying and the complexity of legislation. The information system also showed low quality and effectiveness indicators in making decisions.
