Summary:ABSTRACT: For the diagnosis of heart failure, it is necessary to find the symptoms and signs of the disease as well as objective evidence of a structural or functional heart abnormality. Methodology: We followed the steps defined in the Methodological Guide for the Preparation of Clinical Practice Guidelines and Economic Evaluation in the General System of Social Security of the Colombian Ministry of Health and Social Protection to develop the clinical practice guidelines based on evidence (GPC). Results: In this issue we present the recommendations to establish such diagnosis. Conclusion: In a substantial proportion of patients, diagnosis of heart failure is established on the basis of the clinical picture, together with the information provided by electrocardiogram and chest radiograph. In dubious cases other diagnostic tools should be used to establish such diagnosis with a low degree of uncertainty.
