The suggested treatment to treat this condition is based on the control of factors and risk and the use of fluoride-based therapies. Objective: To establish the influence of risk factor management on the appearance of caries early childhood. Methods: A systematic literature review was performed by searching for articles scientists in the Pubmed, Lilacs, Proquest, Ebsco, Web of Science, Scopus, ScienceDirect and Clinical Key databases. Titles, summaries and full texts were read. The quality and validity of the studies were determined using the Consort, Strobe and Prisma checklists. The articles were classified according to the level of evidence and the degrees of recommendation through SIGN. Results: Most of the articles report that the inadequate management of risk factors such as bad oral hygiene practices; the high consumption of sugars and the low socioeconomic level, were significantly related to the presence of early childhood caries. In-office therapies for the prevention of dental caries are effective, however, better results are seen when combined with oral hygiene education and diet intervention. Conclusion: Fluoride-based therapeutics and the use of pit and fissure sealants must be accompanied by an adequate management of risk factors to reduce the prevalence of early childhood caries. Key words: Dental caries, risk factor, temporary dentition, sealants, fluorine varnish.Antecedentes: La caries de infancia temprana (CIT) se define como la presencia de uno o más dientes con pérdida de estructura dental debido a caries y/o la presencia de obturaciones en niños menores de 71 meses. El tratamiento sugerido para tratar esta condición se basa en el control de factores de riesgo y la utilización de terapias a base de fluoruros. Objetivo: Establecer la influencia del manejo de los factores de riesgo en la aparición de caries de infancia temprana. Métodos: Se realizó una revisión sistemática de la literatura mediante la búsqueda de artículos científicos en las bases de datos Pubmed, Lilacs, Proquest, Ebsco, Web of Science, Scopus, ScienceDirect y Clinical Key. Se leyeron títulos, resúmenes y textos completos. Se determinó la calidad y validez de los estudios por medio de las listas de chequeo Consort, Strobe y Prisma. Se clasificaron los artículos según el nivel de evidencia y los grados de recomendación a través de SIGN. Resultados: La mayoría de los artículos reportan que el manejo inadecuado de factores de riesgo como, las malas prácticas de higiene oral; el alto consumo de azúcares y el bajo nivel socioeconómico, se relacionaron significativamente con la presencia caries de infancia temprana. Las terapias realizadas en consultorio para la prevención de caries dental logran ser efectivas, sin embargo, se observan mejores resultados si se combinan con educación en higiene oral e intervención en la dieta. Conclusión: Las terapéuticas a base de fluoruros y la utilización de sellantes de fosas y fisuras deben ir acompañadas de un adecuado manejo de factores de riesgo para reducir la prevalencia de caries de infancia temprana.
