description El uso del microscopio quirúrgico en la cirugía periapical permite realizar osteotomías y apicectomías más conservadoras, permitiendo una manipulación más cuidadosa de los tejidos, lo que se traduce en una cicatrización y reparación más predecible, aumentando la tasa de éxito a largo plazo gracias a su uso. Este estudio observacional descriptivo muestra la evolución de 15 casos que fueron sometidos a Microcirugía Endodóntica (ME) utilizando Biocerámicos Endosequence® y Biodentine™, los cambios óseos se evaluaron mediante tomografía de rayo de cono CBCT a los 6, 12, 24 y 36 meses. Para determinar el proceso de cicatrización ósea se midió el ancho, alto, profundidad y perímetro de la lesión. En los casos 1, 2 y 3 se evidencia un cambio volumétrico favorable a los 12 meses posterior al procedimiento quirúrgico con reducción de la lesión y cicatrización completa que se observa hasta los 36 meses, en los casos 4, 5, 6 y 7 el cambio volumétrico se observó desde el sexto mes manteniendo su proceso de cicatrización completa hasta los 24 meses. En los casos 8, 9, 10, 11, 12, 13, 14 y 15 se observó una disminución de la zona hipodensa a partir de tercer mes con un proceso de cicatrización ósea favorable que se mantuvo durante el periodo de observación. Conclusiones: Se observa que los 15 casos presentaron cambios óseos en perímetro y área durante el periodo de evaluación mediante CBCT.

Este estudio observacional descriptivo muestra la evolución de 15 casos que fueron sometidos a Microcirugía Endodóntica (ME) utilizando Biocerámicos Endosequence® y Biodentine™, los cambios óseos se evaluaron mediante tomografía de rayo de cono CBCT a los 6, 12, 24 y 36 meses. Para determinar el proceso de cicatrización ósea se midió el ancho, alto, profundidad y perímetro de la lesión. En los casos 1, 2 y 3 se evidencia un cambio volumétrico favorable a los 12 meses posterior al procedimiento quirúrgico con reducción de la lesión y cicatrización completa que se observa hasta los 36 meses, en los casos 4, 5, 6 y 7 el cambio volumétrico se observó desde el sexto mes manteniendo su proceso de cicatrización completa hasta los 24 meses. En los casos 8, 9, 10, 11, 12, 13, 14 y 15 se observó una disminución de la zona hipodensa a partir de tercer mes con un proceso de cicatrización ósea favorable que se mantuvo durante el periodo de observación. Conclusiones: Se observa que los 15 casos presentaron cambios óseos en perímetro y área durante el periodo de evaluación mediante CBCT.The Endodontic Microsurgery allows more conservative osteotomies and apicectomies, there is better tissue manipulation, getting as result a more predictable cicatrization and reparation, which increases the long-term success rate. This descriptive observational study shows the evolution of 15 teeth’s in 10 patients who were underwent to a Endodontic Microsurgery (ME) using bioceramics Endosequence® and Biodentine ™; The bone changes were evaluated by CBCT cone ray tomography at 3, 6, 12, 24 and 36 months. To determine the evolution of the lesion, the area and perimeter of the lesion were measured. A decrease in the hypodense zone was observed from the third month, which was maintained during the observation period.
