Summary:There is currently no pharmaceutical care program for women with Hormonal Supplementation and Blood Hypertension concomitant with non-insulin dependent Diabetes Mellitus, so these women have a high risk of presenting Drug Related Problems and Negative Drug Reactions, which The search for pharmaceutical care programs is to reduce these risks. The present work aims to propose a program of pharmaceutical care for women with Hormonal Supplementation Substitute and Hypertension concomitant with non-insulin dependent Diabetes Mellitus, starting from the design of activities such as Induction on demand, Education and Specific Protection according to the DADER method Based on a Markov model; Where different stages were identified according to the natural history of the disease through time. Emphasizing the figure of the pharmaceutical chemist, as the professional qualified to carry out the implementation of pharmaceutical care, according to law 212 of 1995 and decree 2200 of 2005, with the aim of improving the quality of life of these patients
