Este trabajo monográfico revisa la historia y actualiza el debate contemporáneo en relación con el uso del concepto de muerte encefálica y sus implicaciones bioéticas. En el capítulo 1 se presenta el antecedente histórico de la posibilidad de errar en el diagnóstico, al declarar a una persona como muerta; luego, se identifican los avances desarrollados a finales del siglo XIX y principios del XX en cuanto a tecnología médica, que permitieron mantener con vida a pacientes con lesiones neurológicas catastróficas, es decir, un nuevo tipo de paciente. Esta nueva cohorte fue catalogada como muerta, según el argumento principal propuesto por el comité ad hoc de la Escuela de Medicina de la Universidad Harvard (Ad Hoc Committee of the Harvard Medical School, 1968). En este capítulo, además, se citan a los primeros autores que dieron cuenta del fenómeno clínico descrito como coma irreversible que posteriormente se hizo conocer como muerte encefálica.
