Background and objectives: Sarcopenia is a progressive and generalized skeletal muscle disorder that involves accelerated loss of muscle mass and function. The European Working Group on Sarcopenia recommends the use of the skeletal muscle mass index, which is muscle mass adjusted for height for diagnosis. However, height loss is a frequent finding in older people. The present study aims to establish differences in the diagnosis of sarcopenia in women over 65 years of age according to the use of standing height or surrogate height measurements. Methods: A cross-sectional relational study was conducted in a total of 62 women over 65 years of age from the city of Manizales, Colombia. Body composition analysis was performed using bioimpedance measurement, dynamometry, and anthropometric measurements that included standing height, heel-knee length measurement, and the height recorded on the identity card. The skeletal muscle mass index was calculated taking each of the heights and a diagnosis of sarcopenia was made to those with decreased skeletal muscle mass index and low grip strength. Results: The prevalence of sarcopenia by standing height was (9.7%), by height calculated by heel-knee length (11.3%) and by ID card height (14.5%) (p = 0.096972), the study population had an average age of 78.5 years, standing height of 1.52 mt, calculated by heel-knee length of 1.50 mt and recorded in ID card height of 1.57 mt. The 3 methods of measuring the IMME were analyzed with a result of 8.92 kg/m2 for the standing IMME, 9.063 kg/m2 for the IMME using the heel-knee length, and 8.58 kg/m2 for the IMME using the height recorded on the ID card, with significant differences (p < 0.001). Conclusion: Differences were identified between the height measurements analyzed (foot measurement, estimated by heel-knee measurement and recorded on the citizenship card), with the one recorded on the ID card presenting the highest prevalence of sarcopenia. Although differences were observed in the prevalence of sarcopenia according to the height measurement method, the Cochran test did not show statistically significant differences between the three methods.
