Efectos fisiológicos favorables de un protocolo de hidroterapia de colón (PHTC) en mujeres adultas jóvenes con sobrepesoMicrobiota intestinalSobrepesoHidroterapia de colonDislipidemiaMetainflamaciónCiencias médicasIlustraciones, fotosspa:Introducción: en lo que ha corrido del presente siglo, la ciencia ha demostrado una asociación clara entre la composición de la microbiota intestinal y diversos tipos de enfermedades crónicas no transmisibles, de manera especial con la hiperadiposidad corporal (sobrepeso y obesidad). La hiperadiposidad abdominal es considerada el factor principal alrededor del cual gravita el síndrome metabólico (aumento de la presión arterial, niveles altos de glucosa en sangre y niveles anormales de colesterol o triglicéridos), con su consecuente riesgo elevado de enfermedad cardiovascular. La Organización Mundial de la Salud ha propuesto un esquema general de manejo del sobrepeso y la obesidad basado en tres componentes fundamentales: a) ajuste de la dieta, b) incremento del ejercicio físico y c) intervenciones de tipo cognitivo conductual. Sin embargo, lo que se ha logrado evidenciar, es que los tratamientos de adelgazamiento se han mostrado, en alto grado, ineficaces en el mediano y largo plazo. Actualmente, se considera que el colon es clave para el bienestar, la salud y la enfermedad, y la modulación de la microbiota intestinal se está convirtiendo en un objetivo terapéutico para algunas afecciones médicas. Las intervenciones para esta modulación van desde las tradicionales basadas en dieta y ejercicio, hasta los trasplantes fecales intestinales, pasando por el consumo de pro- pre y simbióticos. El objetivo principal del presente estudio consistió en medir los efectos de un tratamiento de hidroterapia del colon, considerado como una alternativa de modulación de la microbiota intestinal humana, sobre una serie de variables fisiológicas físicas, químicas y biológicas asociadas al sobrepeso/obesidad. Métodología: se propuso un protocolo de limpieza de colon de 6 días para modular la composición de la microbiota intestinal y obtener mejoras visibles en un conjunto seleccionado de 22 variables fisiológicas asociadas al sobrepeso y la obesidad: 11 físicas, 9 químicas y 2 biológicas (razón Firmicutes/Bacteroidota , RF/B, y la prevalencia de Akkermansia sp). Todas las participantes, luego de leer la hoja de información del proyecto y firmar el consentimiento informado, fueron examinadas en tres tiempos distintos (T1, T2, T3 y T4). Se utilizó una prueba t de Student para comparar las medias de cada variable entre ambos grupos en T1, T2 T3 y T4, mientras que se aplicó un ANOVA de medidas repetidas para los puntos temporales para la comparación de las medias de cada variable en cada grupo en los tres momentos diferentes. La variable de respuesta fue cada una de las variables fisiológicas medidas (peso, % de PMGC, etc.). Cuando el ANOVA mostró diferencias, se utilizó la prueba de Bonferroni para determinar dónde estaban esas diferencias. En ambos casos, los valores p < 0,05 se consideran estadísticamente significativos. La correlación entre las medias de cada una de las variables fisiológicas físicas y químicas estudiadas con las medias del biomarcador biológico RF /B se realizó con la prueba de correlación de rangos de Spearman. Resultados: aquí, mostramos que los cambios favorables en todas las variables seleccionadas están presentes en el grupo experimental inmediatamente después del tratamiento y un mes después, sin más intervención, y que los cambios observados en las variables físicas y químicas están altamente correlacionados con los cambios en la RF/B. Hubo correlaciones estadísticamente significativas entre la mayoría de las variables fisiológicas físicas y químicas con la RF/B, excepto para tres de ellas: FCR, PAD, PAS. Las tres variables que muestran una correlación negativa son aquellas cuyo incremento puede considerarse beneficioso: FAMM, PWMSA y HDL. Discusión: el sobrepeso o la obesidad, al igual que la mayoría de las enfermedades crónicas no transmisibles (ECNTs), se asocian a hábitos de vida poco saludables, que producen alteraciones en el equilibrio del MBIH (disbiosis). Dado que los hábitos de vida son modificables, las ECNTs se consideran prevenibles. La modulación de la microbiota al inicio de una intervención de adelgazamiento para restablecer ese equilibrio (eubiosis) podría resultar muy útil como coadyuvante de los cambios en la dieta y la actividad física y mejorar su eficacia. Los resultados preliminares aportados por este trabajo de investigación son muy alentadores y merecen un futuro estudio clínico más amplio para obtener un sólido apoyo estadístico a esta propuesta. Recomendaciones: a partir de los resultados obtenidos en el presente estudio, se sugiere: a) realizar nuevos estudios con un número mayor de participantes; b) incluir un programa de actividad física complementario al Protocolo de Hidroterapia de Colon; c) explorar a mayor profundidad los cambios producidos en la composición de la microbiota intestinal; d) considerar otros grupos de posibles participantes del estudio.eng:Introduction: in the course of the present century, science has demonstrated a clear association between the composition of the intestinal microbiota and various types of chronic non-communicable diseases, especially with body hyperadiposity (overweight and obesity). Abdominal hyperadiposity is considered the main factor around which the metabolic syndrome (increased blood pressure, high blood sugar levels and abnormal cholesterol or triglyceride levels) gravitates, with its consequent elevated risk of cardiovascular disease. The World Health Organization has proposed a general management scheme for overweight and obesity based on three fundamental components: a) dietary adjustment, b) increased physical exercise and c) cognitive-behavioral interventions. However, what has become evident is that weight loss treatments have been shown, to a high degree, to be ineffective in the medium and long term. The colon is now considered key to wellness, health and disease, and modulation of the gut microbiota is becoming a therapeutic target for some medical conditions. Interventions for this modulation range from traditional ones based basically on diet and exercise, to intestinal fecal transplants (microbiota/microbiome), to the consumption of pro- pre and symbiotics. The main objective of the present study consisted in measuring the effects of a colon hydrotherapy treatment on some physiological variables (physical, chemical and biological) associated whith being overweight or obese, as a possible alternative for human intestinal microbiota modulation. Methodology: a 6-day colon cleansing protocol was proposed to modulate the intestinal microbiota and obtain visible improvements in a selected set of 22 physiological variables associated with overweight and obesity: 11 physical, 9 chemical and 2 biological (Firmicutes/Bacteroidetes , ratio, and the prevalence of Akkermansia sp). All participants, after reading the project information sheet and signing the informed consent, were examined at three different times (T1, T2, T3 and T4). A Student's t-test was used to compare the means of each variable between both groups at T1, T2, T3 and T4, while a repeated measures ANOVA for time points was applied for the comparison of the means of each variable in each group at the three different times. The response variable was each of the physiological variables measured (weight, % PBFM, etc.). When the ANOVA showed differences, the Bonferroni test was used to determine where those differences were. In both cases, p-values < 0.05 are considered statistically significant. The correlation between the means of each of the physical and chemical physiological variables studied with the means of the biological biomarker F/BR was performed with Spearman's rank correlation test. Results: here, we show that favorable changes in all selected variables are present in the experimental group immediately after treatment and one month later, without further intervention, and that the observed changes in the physical and chemical variables are highly correlated with changes in F/BR. There were statistically significant correlations between most of the physical and chemical physiological variables with RF/B, except for three of them: RCR, DBP, SBP. The three variables showing a negative correlation are those whose increase can be considered beneficial: MHGS, MWSRT and HDL. Discussion: overweight or obesity, like most chronic noncommunicable diseases (NCDs), are associated with unhealthy lifestyle habits, which produce epigenetic changes and alterations in the balance of the IM (dysbiosis). Since lifestyle habits are modifiable, NCDs are considered preventable. Modulation of the microbiota at the beginning of a weight loss intervention to restore this balance (eubiosis) could be very useful as an adjuvant to changes in diet and physical activity and improve their efficacy. The preliminary results provided by this research work are very encouraging and deserve a future larger clinical study to obtain a solid statistical support for this proposal.
