description La citología aspirativa con aguja fina es un procedimiento usado en el análisis de lesiones mamarias. En este artículo se muestran los hallazgos obtenidos por la revisión de casos durante la década de 1990 a 1999, en el Hospital de Caldas, con el fin de determinar el grado de confiabilidad del diagnóstico citológico, tomando el estudio anatomopatológico como la prueba de oro; se revisaron casos de mujeres con lesiones de mama, a quienes se les había realizado una o más citologías aspirativas y uno o más estudios anatomopatológicos. Se encontraron 132 (58.14%) mujeres con neoplasias malignas y 95 (41.85%) con lesiones benignas. El análisis de las muestras adecuadas mostró sensibilidad del 93%, especificidad del 76%, valor predictivo del test positivo del 85% y valor predictivo del test negativo del 89%. Los falsos positivos fueron 18 (9.62%) casos que correspondían especialmente a error en la interpretación citológica y los falsos negativos fueron 7 (3,74%) casos, cuyos extendidos fueron muestras inadecuadas. En total hubo 53 citologías (23.34%) inadecuadas para diagnóstico, un 83% de ellas debido a ausencia o escasez de la muestra, y un 17% por extendidos gruesos o hemorrágicos. Las muestras inadecuadas fueron la causa de repetición de un número importante de punciones y a su vez la causa de falsos resultados, especialmente negativos. El número de muestras inadecuadas fue mayor cuando el procedimiento lo realizó personal médico sin entrenamiento y bajó cuando el mismo adquirió practica.

In this article the discoveries obtained by the revision of cases during the decade from 1990 to 1999 in the Hospital of Caldas are shown, with the purpose of determining the degree of dependability of the cytologic diagnosis, taking the anatomopathologic study like the test of gold; cases were revised of women with pathology of her breast who had been carried out one or more aspirative cytologies and one or more anatomopathologic studies. There were 132 (58.14%) malignant neoplasms and 95 (41.85%) benign injuries. The analysis of the appropriate samples showed sensibility of 93%, specificity of 76%, predictive value of the positive test of 85% and predictive value of the negative test of 89%. The false positives were 18 (9.62%) cases that corresponded especially to error in the cytologic interpretation and the false negatives were 7 (3,74%) cases whose samples were inadequate. There were 53 (23.34%) inadequate cytologies for diagnosis, 83% of them due to lack or shortage of the sample, and 17% due to thick or hemorrhagic samples. The inadequate samples were the cause of repetition of an important number of punctions and in turn the cause of false results, specially negative.
