description La terapia antiretroviral es el principal determinante en la supervivencia de los pacientes con VIH; el entorno social, el manejo de las comorbilidades y la facilidad de acceso al sistema de salud, también modifican el desenlace. Se realizó un estudio longitudinal de tipo observacional analítico, a partir de una cohorte de pacientes que asistieron durante 2005 - 2016 a un programa de atención integral de VIH/Sida de la ciudad de Bogotá. Se evaluaron datos de 1105 pacientes, el promedio de edad de inicio de tratamiento fue 35.0±9.5 años y la mortalidad fue del 3.3% (n=37). Se encontró diferencia significativa en la sobrevida acumulativa por grupos etarios (p=0.001, Prueba Log-Rank), mostrando mayor sobrevida en los grupos de menor edad. Se evidenció mayor riesgo significativo de mortalidad en los fumadores (p=0.015), sin inmunidad para hepatitis B (p<0.001), bajo peso (p=0.016), con recuento menor a 200 cel/mm3 (p=0.001) y aquellos con cargas virales mayores a 500 copias/ml (p<0.001). No se encontraron diferencias significativas según tipo de hospitalizaciones presentadas ni patología asociada. Se presentó una disminución significativa de la mortalidad en aquellos con inmunidad frente a la hepatitis B (p=0,006). El VIH es una enfermedad crónica, la supervivencia en estos pacientes obedece a variables diferentes al tratamiento y el estado inmunovirológico. Explorar la influencia de factores sociodemográficos, comorbilidades, el estado clínico y la atención adecuada en programas de atención integral en VIH que garanticen una atención completa, influye de manera positiva en la expectativa de vida de las personas que viven con VIH/SIDA.

A longitudinal analytical observational study was conducted, from a cohort of patients who attended during 2005 - 2016 to a comprehensive HIV / AIDS care program in the city of Bogotá. Data from 1105 patients were evaluated, the average age of onset of treatment was 35.0 ± 9.5 years and the mortality was 3.3% (n = 37). A significant difference was found in the cumulative survival by age groups (p = 0.001, Log-Rank test), showing greater survival in the younger age groups. There was a greater significant risk of mortality in smokers (p = 0.015), without immunity for hepatitis B (p <0.001), low weight (p = 0.016), with a count lower than 200 cel / mm3 (p = 0.001) and those with viral loads greater than 500 copies / ml (p <0.001). No significant differences were found according to type of hospitalizations presented or associated pathology. There was a significant decrease in mortality in those with immunity against hepatitis B (p = 0.006). HIV is a chronic disease, survival in these patients is due to variables other than treatment and immuno-virological status.
