“Jersey Finger" o “dedo del jugador” de rugby, es una patología en la cual se pierde la función en flexión de la articulación interfalángica distal de cualquier dedo afectado de la mano. Existen cinco patrones de lesión, descritos por Leddy y Packer, en donde los signos clínicos y síntomas son similares, con diferencias en los hallazgos quirúrgicos. El tipo VI es la presentación más infrecuente. Presentamos el caso de un paciente masculino, adulto joven, que por trauma dado por caída, se documenta lesión jersey finger tipo IV, con hallazgos intraquirúrgicos de la fractura desplazada, diafisaria, epifisaria proximal de la falange distal, asociado a una desinserción completa con retracción de la inserción del tendón flexor profundo de los dedos hasta zona II Flexora. Con los hallazgos pre e intraquirúrgicos se realiza un manejo por separado de las lesiones, que consiste en la reconstrucción ósea y tendinosa, en un solo tiempo quirúrgico, diferenciándonos de otros procedimientos quirúrgicos propuestos, dando como resultado una reconstrucción articular completa, que, asociado a la reinserción del flexor profundo de forma efectiva y dinámica permitió al paciente tener una adecuada funcionalidad final. Hacemos una propuesta de manejo integral en donde la fijación tras articular con clavos, perdiendo la funcionalidad articular, manejo usual, no fue necesario y donde sugerimos una reconstrucción tendinosa y ósea separada, en un tiempo quirúrgico. Esto asociado a un tiempo de rehabilitación activa, nos da como resultado una funcionalidad articular similar a la normal de los demás dedos. |

There are five patterns of injury, described by Leddy and Packer, where clinical signs and symptoms are similar, with differences in surgical findings. Type VI is the rarest presentation. We present the case of a young male patient who has a fall trauma injury in his hand, documented a type IV jersey finger injury, with intra-surgical findings of a displaced, diaphyseal, proximal epiphyseal fracture of the distal phalanx, associated with complete disinsertion of the flexor digitorum profundus tendon to flexor zone II with retraction of their fibers. With the pre- and intra-surgical findings, a separate management of the injuries is carried out, which consists of bone and tendon reconstruction, in a single surgical time, differentiating us from other proposed surgical procedures. As a result we had a complete joint reconstruction, which, associated with reinsertion of the deep flexor in an effective and dynamic way allowed the patient to have adequate final functionality. We make a comprehensive management proposal where fixation of the bone and tendon injury appart has a good outcome, the usual management, was not necessary and where we suggest a separate tendon and bone reconstruction, in one surgical time.
