description	El trasplante simultáneo de hígado y riñón (TSHR) es un procedimiento complejo, infrecuente, que constituye el tratamiento de elección en pacientes con falla hepática y renal concurrentes en estadío terminal[1]. Este estudio es descriptivo, observacional y tiene como objetivo analizar la experiencia con TSHR en la Fundación Cardioinfantil desde el 1 de enero del 2005 hasta el 31 de diciembre del 2021. En cuanto a la metodología, luego de aprobación institucional, se realizó una revisión retrospectiva de los casos de TSHR efectuados entre 2005 y 2021, recolectando parámetros pre, intra y postoperatorios. Se encontró que durante el periodo de estudio se practicaron 31 TSHR que corresponde al 4.1% de los trasplantes hepáticos y a un 5.9% de los trasplantes renales, el número de procedimientos permaneció estable a lo largo del tiempo. Del total de los casos, 8 eran pediátricos. La mediana del MELD (Model for End-stage Liver Disease) fue 20.5 puntos (RIC 14-22.8). La etiología hepática más frecuente en adultos fue la cirrosis alcohólica (52.1%) y en la población infantil la fibrosis hepática congénita (62.5%). La enfermedad renal más frecuente en adultos fue la nefropatía diabética (52.1%), mientras que en niños primó la nefronoptisis (37.5%). 100% de los adultos obtuvo órganos de donantes cadavéricos, en pacientes pediátricos 50% recibió órganos de donante cadavérico y 50% de donante vivo. Durante la hospitalización se presentó en adultos 1 falla y 1 rechazo del injerto renal, sin observarse pérdidas hepáticas. Dos pacientes pediátricos fallecieron a causa de sepsis y falla multiorgánica. En conclusión, este reporte preliminar muestra que el TSHR en nuestra institución tiene resultados clínicos satisfactorios comparables a los reportados en la literatura

This study is descriptive and observational, and its objective is to analyze the experience with TSHR at Fundación Cardioinfantil from January 1, 2005, to December 31, 2021. Regarding the methodology, after institutional approval, a retrospective review was carried out. of TSHR cases eliminated between 2005 and 2021, collecting pre, intra, and postoperative parameters. It was found that during the study period, 31 TSHR were performed, corresponding to 4.1% of liver transplants and 5.9% of kidney transplants; the number of procedures remained stable over time. Of the total cases, 8 were pediatric. The median of the MELD (Model for End-stage Liver Disease) was 20.5 points (IQR 14-22.8). The most common liver etiology in adults was alcoholic cirrhosis (52.1%) and in the child population congenital liver fibrosis (62.5%). The most common kidney disease in adults was diabetic nephropathy (52.1%), while nephronopthisis prevailed in children (37.5%). 100% of adults obtained organs from cadaveric donors, in pediatric patients 50% received organs from cadaveric donors, and 50% from living donors. During hospitalization, 1 renal graft failure and 1 rejection occurred in adults, without observing liver losses. Two pediatric patients died from sepsis and multiple organ failure.
