description El cáncer de laringe es una patología altamente mórbida con un impacto muy importante en el estado nutricional del paciente, que así mismo influye en el desenlace posoperatorio de los pacientes candidatos a laringectomía total, sin embargo, sin conductas estandarizadas en el ámbito nutricional aplicables a estos pacientes. Objetivo: Realizar un consenso con recomendaciones basadas en la evidenica para la práctica clínica en torno a las conductas nutricionales empleadas en el preoperatorio y postoperatorio de pacientes adultos sometidos a laringectomías totales en Colombia. Métodos: Se realizo una revisión sistemática de la literatura para la construcción de recomendaciones evaluadas por 14 cirujanos de cabeza y cuello del país. Los miembros del panel recibieron 16 recomendaciones basadas en la evidencia las cuales fueron calificadas por los expertos utilizando la escala Likert de 5 puntos además de la posibilidad de una respuesta abierta. Se definió consenso de acuerdo o desacuerdo con un porcentaje de respuestas unánimes en uno o dos ítems (negativo o positivo) mayor al 70%. Resultados: 14 cirujanos de cabeza y cuello aceptaron participar en el consenso. Un total de 10 recomendaciones alcanzaron consenso en primera vuelta, las 6 recomendaciones restantes se sometieron a segunda vuelta en la cual 3 de estas consiguieron consenso. Como resultado final se obtuvieron 13 recomendaciones finales basadas en la literatura científica en torno a las conductas nutricionales perioperatorias en laringectomía total. Conclusiones: El tratamiento nutricional integral de los pacientes sometidos a laringectomía total implica un enfoque polifacético y multidisciplinario, que incluye la evaluación preoperatoria, inicio de vía oral temprana teniendo en cuenta la individualización de cada caso, inicio de las vías de alimentación adecuadas y consideración de la vía oral como primera opción. Este enfoque, respaldado por las pruebas científicas disponibles, pretende optimizar el estado nutricional del paciente y favorecer su recuperación y bienestar a largo plazo.

Objetivo: Realizar un consenso con recomendaciones basadas en la evidenica para la práctica clínica en torno a las conductas nutricionales empleadas en el preoperatorio y postoperatorio de pacientes adultos sometidos a laringectomías totales en Colombia. Métodos: Se realizo una revisión sistemática de la literatura para la construcción de recomendaciones evaluadas por 14 cirujanos de cabeza y cuello del país. Los miembros del panel recibieron 16 recomendaciones basadas en la evidencia las cuales fueron calificadas por los expertos utilizando la escala Likert de 5 puntos además de la posibilidad de una respuesta abierta. Se definió consenso de acuerdo o desacuerdo con un porcentaje de respuestas unánimes en uno o dos ítems (negativo o positivo) mayor al 70%. Resultados: 14 cirujanos de cabeza y cuello aceptaron participar en el consenso. Un total de 10 recomendaciones alcanzaron consenso en primera vuelta, las 6 recomendaciones restantes se sometieron a segunda vuelta en la cual 3 de estas consiguieron consenso. Como resultado final se obtuvieron 13 recomendaciones finales basadas en la literatura científica en torno a las conductas nutricionales perioperatorias en laringectomía total. Conclusiones: El tratamiento nutricional integral de los pacientes sometidos a laringectomía total implica un enfoque polifacético y multidisciplinario, que incluye la evaluación preoperatoria, inicio de vía oral temprana teniendo en cuenta la individualización de cada caso, inicio de las vías de alimentación adecuadas y consideración de la vía oral como primera opción. Este enfoque, respaldado por las pruebas científicas disponibles, pretende optimizar el estado nutricional del paciente y favorecer su recuperación y bienestar a largo plazo.Laryngeal cancer is a highly morbid pathology with a very important impact on the nutritional status of the patient, which likewise influences the postoperative outcome of patients who are candidates for total laryngectomy, however without standardized behaviors in the nutritional field applicable to these patients. Objective: To make a consensus with recommendations based on scientific evidence for clinical practice regarding the nutritional behaviors used preoperatively and postoperatively in adult patients undergoing total laryngectomy in Colombia. Methods: A Delphi consensus was reached among 14 head and neck surgeons in the country. The panel members received 16 evidence-based recommendations which were rated by the experts using the 5-point Likert scale plus the possibility of an open response. Consensus was defined as agreement or disagreement with a percentage of unanimous responses on one or two items (negative or positive) greater than 70%. Results: A total of 14 head and neck surgeons agreed to participate in the consensus. A total of 10 recommendations reached consensus in the first round, the remaining 6 recommendations were submitted to a second round in which 3 of them reached consensus. As a final result, 13 final recommendations were obtained regarding perioperative nutritional behaviors in total laryngectomy. Conclusions: Comprehensive nutritional management of patients undergoing total laryngectomy involves a multifaceted and multidisciplinary approach, including preoperative assessment, early oral route initiation taking into account the individualization of each case, initiation of appropriate feeding routes and consideration of the oral route as the first option .
