Introducción: La sepsis y el shock séptico constituyen un gran problema de salud, la mortalidad es muy alta, y la principal causa de LRA. La identificación temprana de la hipoperfusión tisular es claves en el tratamiento de pacientes con sepsis. Actualmente el lactato se utiliza para evaluar hipoperfusión tisular con muchos limitantes, se están explorando marcadores adicionales como PvaCO2/CavO2. Materiales y métodos: Estudio observacional de una cohorte retrospectiva, incluyendo pacientes mayores de 18 años con diagnóstico de sepsis y/o choque séptico atendidos en la UCI durante un año, Se describe el valor pronóstico del lactato e índice PvaCO2/CavO2 al momento del diagnóstico de sepsis y 24 horas, asociado con las variables clínicas de ingreso y a las 48 horas, y su relación con la mortalidad a 28 días. Se realizo un análisis bivariado de las variables de importancia. Resultados: Los pacientes del estudio (207) tenían a altos índices de SOFA (10 puntos) y APACHE (26 puntos), choque séptico el 86%, COVID el 82.6%, y lesión renal aguda al ingreso en el 53% y a las 48 horas el 56% de los pacientes. Antes de las 48 horas fallecieron 23.16% de los pacientes, con una mortalidad a los 28 días del 97,1%. El lactato y PvaCO2/CavO2 en promedio estuvo debajo de los valores de hipoperfusión. Conclusión La lesión renal a las 48 horas se asocia a mayor mortalidad a los 28 días. Los pacientes con COVID tenían mayores alteraciones en el intercambio gaseoso. El Lactato y PCO2/CavO2 no se encontró correlación en los pacientes que tenían sepsis. |

La identificación temprana de la hipoperfusión tisular es claves en el tratamiento de pacientes con sepsis. Actualmente el lactato se utiliza para evaluar hipoperfusión tisular con muchos limitantes, se están explorando marcadores adicionales como PvaCO2/CavO2. Materiales y métodos: Estudio observacional de una cohorte retrospectiva, incluyendo pacientes mayores de 18 años con diagnóstico de sepsis y/o choque séptico atendidos en la UCI durante un año, Se describe el valor pronóstico del lactato e índice PvaCO2/CavO2 al momento del diagnóstico de sepsis y 24 horas, asociado con las variables clínicas de ingreso y a las 48 horas, y su relación con la mortalidad a 28 días. Se realizo un análisis bivariado de las variables de importancia. Resultados: Los pacientes del estudio (207) tenían a altos índices de SOFA (10 puntos) y APACHE (26 puntos), choque séptico el 86%, COVID el 82.6%, y lesión renal aguda al ingreso en el 53% y a las 48 horas el 56% de los pacientes. Antes de las 48 horas fallecieron 23.16% de los pacientes, con una mortalidad a los 28 días del 97,1%. El lactato y PvaCO2/CavO2 en promedio estuvo debajo de los valores de hipoperfusión. Conclusión La lesión renal a las 48 horas se asocia a mayor mortalidad a los 28 días. Los pacientes con COVID tenían mayores alteraciones en el intercambio gaseoso. El Lactato y PCO2/CavO2 no se encontró correlación en los pacientes que tenían sepsis.Introduction: Sepsis and septic shock are a major health problem, mortality is very high, and the main cause of AKI. Early identification of tissue hypoperfusion is key in the treatment of patients with sepsis. Lactate is currently used to assess tissue hypoperfusion with many limitations, additional markers such as PvaCO2/CavO2 are being explored.
