description	Introducción. La hipertensión aumenta considerablemente el riesgo de sufrir enfermedades cardiacas y actualmente la cardiopatía isquémica es la principal causa de muerte en todo el mundo. Enfermería puede ayudar a disminuir esa estadística, se buscó respuesta entonces a la pregunta: ¿Cuáles son las acciones de enfermería para la intervención de las barreras que limitan la educación, autocuidado y seguimiento de los pacientes hipertensos en el Programa de riesgo de cardiovascular?, por tanto el objetivo es determinar las acciones de enfermería para la intervención de las barreras que mayor incidencia tienen en la educación, autocuidado y seguimiento de los pacientes hipertensos del programa de riesgo cardiovascular; los materiales y métodos utilizados consisten en el diseño metodológico de revisión narrativa, en cuanto a los resultados arrojados se destaca que entre las barreras que más afectan la adherencia al tratamiento son la edad, sexo, falta de disponibilidad de pacientes y personal de salud, gastos, tiempo, comunicación ineficaz, la familia, falta de apoyo, ocupación, nivel económico, alfabetización, desconocimiento de la enfermedad, participación de múltiples especialistas, estilos de vida, y entre las acciones que señalaron eficacia fueron las herramientas digitales, consultas con acompañante, reuniones en equipo de los profesionales, consultas triádicas, aplicar enfoque interpersonal, educación para la salud, no utilizar términos especializados y complicados e intervenciones basadas en modelos como el de creencia en salud (HBM) y campañas de concientización. Discusión, la educación, autocuidado y seguimiento adecuado de los pacientes hipertensos son elementos fundamentales para mejorar los resultados clínicos y reducir la carga de la enfermedad. En conclusión, si existen en la literatura intervenciones que han demostrado eficacia para combatir las barreras existentes en los programas, así que un gran paso es que enfermería empiece a implementarlas.

La hipertensión aumenta considerablemente el riesgo de sufrir enfermedades cardiacas y actualmente la cardiopatía isquémica es la principal causa de muerte en todo el mundo. Enfermería puede ayudar a disminuir esa estadística, se buscó respuesta entonces a la pregunta: ¿Cuáles son las acciones de enfermería para la intervención de las barreras que limitan la educación, autocuidado y seguimiento de los pacientes hipertensos en el Programa de riesgo de cardiovascular?, por tanto el objetivo es determinar las acciones de enfermería para la intervención de las barreras que mayor incidencia tienen en la educación, autocuidado y seguimiento de los pacientes hipertensos del programa de riesgo cardiovascular; los materiales y métodos utilizados consisten en el diseño metodológico de revisión narrativa, en cuanto a los resultados arrojados se destaca que entre las barreras que más afectan la adherencia al tratamiento son la edad, sexo, falta de disponibilidad de pacientes y personal de salud, gastos, tiempo, comunicación ineficaz, la familia, falta de apoyo, ocupación, nivel económico, alfabetización, desconocimiento de la enfermedad, participación de múltiples especialistas, estilos de vida, y entre las acciones que señalaron eficacia fueron las herramientas digitales, consultas con acompañante, reuniones en equipo de los profesionales, consultas triádicas, aplicar enfoque interpersonal, educación para la salud, no utilizar términos especializados y complicados e intervenciones basadas en modelos como el de creencia en salud (HBM) y campañas de concientización. Discusión, la educación, autocuidado y seguimiento adecuado de los pacientes hipertensos son elementos fundamentales para mejorar los resultados clínicos y reducir la carga de la enfermedad. En conclusión, si existen en la literatura intervenciones que han demostrado eficacia para combatir las barreras existentes en los programas, así que un gran paso es que enfermería empiece a implementarlas.Introduction. Hypertension significantly increases the risk of heart disease, and ischemic heart disease is currently the leading cause of death worldwide. Nursing can help to reduce this statistic, an answer was then sought to the question: What are the nursing actions for the intervention of the barriers that limit the education, self-care and follow-up of hypertensive patients in the Cardiovascular Risk Program?, therefore the objective is to determine the nursing actions for the intervention of the barriers that have the greatest incidence in education, self-care and follow-up of hypertensive patients in the cardiovascular risk program; The materials and methods used consist of the methodological design of narrative review, as for the results yielded, it is highlighted that among the barriers that most affect adherence to treatment are age, sex, lack of availability of patients and health personnel, expenses, time, ineffective communication, family, lack of support, occupation, economic level, literacy, lack of knowledge of the disease, participation of multiple specialists, lifestyles, and among the actions that indicated effectiveness were digital tools, consultations with companions, team meetings of professionals, triadic consultations, applying interpersonal approach, health education, not using specialized and complicated terms and interventions based on models such as health belief (HBM) and awareness campaigns. Discussion, education, self-care, and adequate follow-up of hypertensive patients are fundamental elements to improve clinical outcomes and reduce the burden of the disease.
